' <br /> July 24, 2002 Page 6 <br />' Hollander Residence Limited Structural Evaluation. <br /> MGI Commission#02162 <br /> for Mr. John Hollander <br />' <br /> Summarv <br />' The intent of this document is to provide a rational reasonable approach to resolving the deficiencies relating to this <br /> residential structure as identified within the City resolution. <br />' These final documents shall be submitted to the city for their approval. Mr. Hollander will take bids on the required <br /> work. A conditional building permit may be requested utilizing this information, conditional upon the continued <br /> substantial adherence to the given work description and schedule. Variation from the given schedule or work coufd <br />' result in loss of the building permit. <br /> This building shall be treated not to detract from any historical significance that it may have. All modifications to the <br /> building shall be reviewed with the owner prior to making such modifications. <br />, Heavy equipment use is prohibited within approximately 10' of the base of the building to reduce the potential of high <br /> lateral loads to the foundation system. This includes equipment and material storage and auto/truck traffic/parking. <br /> t All items being utilized for the construction/repairs of this home shall have their contents disclosed to the owner prior <br /> to use. The owner has allergies to many materials and the final selection of materials shall be made by the owner <br /> prior to use. The use of formaldehyde of any level in any materials is strictly prohibited. <br />' Treated lumber as called for within this report shall be used if it does not contain formaldehyde. Substitutions may <br /> be made upon written submittal and approval of the owner and the engineer of record. <br />' Changes may be made to the given conditions as deemed appropriate and necessary to achieve substantial <br /> completion of the repairs in general conformance with the intentions of this report. <br /> t Fire ratings, means of egress, ADA requirements, new cladding design/attachments or any other factor not <br /> specifically addressed within this report is excluded from this report. <br /> Due to the nature of the structure, additional movement, deflection, cracking etc. may occur with changes to the <br />' house structure. Based upon our review, there were no apparent signs of significant structural distress.
