L. Amendment Request Certificate <br />The form outlines all changes in the approved grant amount, <br />either additions or subtractions, by each contractor. It must <br />be signed by the contractor and the homeowner and approved by <br />the city. A revised bid must be attached. <br />M. Complaint Record <br />This form should be used to document complaints by homeowners which <br />are not resolvable by the grant admiristrator to the satisfaction of <br />both parties, or when the resolution will require a lengthly <br />process. <br />W <br />
