2. Employees shall notify their supervisors as soon as possible <br />after receiving notice to report for jury duty. Each employee <br />will be responsible for ensuring that a Report of Jury Duty and <br />Pay form is completed by the Clerk of Court each week of duty and <br />submitted to the City so the City will be able to determine the <br />amount of compensation due for the period involved. <br />311. LEAVE OF ABSENCE - WITHOUT PAY <br />1. The City Administrator may authorize regular employees to be <br />absent without pay for personal reasons for a period, or periods, <br />not to exceed a total of twenty (20) working days in any calendar <br />year. Leaves of absence without pay in excess of twenty (20) <br />working days in a calendar year must be approved by the City <br />Council. In no instance may a leave of absence exceed 180 <br />calendar days. Employees shall request such leaves of absence <br />without pay in writing well in advance of the date so desired. <br />2. Leaves of absence shall not be given for the purpose of <br />enabling any employee to work for another employer or to engage in <br />any form of self-employment. Any employee who obtains a leave of <br />absence after misrepresenting the purpose therefore shall be <br />disciplined including discharged. <br />312. CANCELLATICN OF LEAVE <br />The City Administrator may cancel any leave of absence, <br />except military leave, at any time upon prior written notice to <br />the employee specifying a date for return to work. <br />313. INSURANCE <br />The City Council shall establish the dollar amount for basic <br />single life insurnac2 and group hospitalization, surgical and <br />medical insurance fo- all qualified regular City employees for <br />those selecting single coverage and a separate dollar amount for <br />those selecting family coverage. <br />314. PUBLIC EMPLOYEES RETIREMENT ASSOCIATION <br />All persons hired for regular positions with the City are <br />members of the Public Employees' Retirement Association unless <br />otherwise exempted by qualification or election provided by law. <br />315. SOCIAL SECURITY <br />Every employee, unless specifically exempted by law, shall be <br />subject to the provisions of the Federal Social SEcurity Old -Age <br />and Survivor's Insurance Program, and deduction to cover such <br />payments shall be made from each payroll. <br />316. PAY DAY <br />The regular pay day for all City employees shall be <br />alternate Wednesdays. <br />6 <br />
