USAGE <br />Usage for vacation and sick leave will be based on the number of <br />hours normally scheduled for that day. (i.e. If the person works <br />8 hc,urs per day, 3 days per week, usage is to be 8 hours for days <br />scheduled. If scheduled 4 hours a day 5 days a week, 4 hours for <br />h day scheduled.) In the case of holidays falling on the <br />:ions regular work day; they shall receive the prorata amount <br />c�-Yrned for a holiday regardless of hours scheduled. If the <br />holiday falls on a day not scheduled to be worked, they shall be <br />paid the prorata holiday hours for that day or may take another <br />day off with the prorata hours earned for that holiday, with <br />the prior written approval of the department head. holiday and <br />work hours will not be paid for the same day. <br />HEALTH -AND -LIFE -INSURANCE <br />The City policy for health insurance for all qualified regular <br />City employees is a contribution of 100% of the cost of employee <br />only coverage for those selecting single coverage, or a maximun, <br />in 1988 of $ 176.00 per month toward the cost of family coverage, <br />and the City policy for basic single life insurance is a <br />contribution of 100% of the cost. <br />For those part-time regular positions earning benefits the City <br />contribution toward basic single life insurance is 100% of the <br />cost and the City contribution rates for health insurance are as <br />follows: <br />City contribution rates: SINGLE -COVERAGE FAMILY_ COVERAGE <br />Under 20 hours - Not available Not available <br />20 - 24 - 100% 55% of maximum <br />24 - 28 - 65% " " <br />28 - 32 - 75% " " <br />32 - 36 - 85% " " <br />36 - 40 - 95% " " <br />Note: The base family insurance contribution rate is 55% of the <br />actual cost or the highest single cost, whichever is greater. <br />For example: the 1988 base contribution would he 55% x $176.00 = <br />$96.80; but the highest single cost is $104.55, so the base <br />contribution would be $104.55. <br />EFFECTIVE DATE <br />This policy is effective immediately upon adoption by the City <br />Council for all hires after 'anuary 1, 1988. <br />FOR_ALI_EMPLOYEES HIRED PRIOR_TO_,TANUARY_1,_1988: <br />The earnings rates for holiday, vacation and sick leave were <br />established at the time of hire as recommended by the office <br />coordinator and no provision wa. made for future adjustments. <br />
