AFMN Community Grant Application with Assurances (GK203)6 <br />Policy <br />Programs/Activities <br />Research <br />Training/Education <br />Communities may also use funds to adopt and implement frameworks such as Age-Friendly <br />Communities, Health Systems, Universities, or Public Health. Communities could also adopt other <br />frameworks or processes to begin or expand age-friendly work. <br />Applicants may view the public announcement (https://mn.gov/dhs/age-friendly-mn/grants/) of the <br />first round of Community Grants, awarded in 2023, for examples of past and/or ongoing projects. <br />Please note that AFMN Grants do not fund direct services. <br />The following table provides examples of the types of expenses that are eligible and ineligible for <br />Community Grant funding. <br />EXAMPLES OF ELIGIBLE AND INELIGIBLE EXPENSES <br />Funding Area Eligible Expense:Ineligible Expense: <br />Connected <br />Communities <br />Develop a plan or strategy for how to <br />utilize an accessible vehicle in the <br />community during off hours <br />Plan the creation and publicity of a <br />volunteer driver program <br />A service that provides rides to older <br />adults <br />Emergency <br />Preparedness, <br />Individual Rights and <br />Safety <br />Developing a community emergency <br />response plan—costs could include <br />consultant fees, community <br />engagement, or developing public <br />awareness campaign. <br />Purchase of tablet devices for <br />individuals to keep. <br />Optimized Health <br />and Longevity <br />Convene partners to collaborate on <br />new strategies for encouraging and <br />supporting youth/others to enter <br />direct care workforce. <br />Conduct research and create <br />recommendations report on healthy <br />aging in a specific cultural community <br />Provide blood pressure screenings <br />Economic Security <br />and Vitality <br />Develop a toolkit and outreach effort <br />to educate employers about the value <br />of older workers <br />Employment counseling for older <br />adults <br />2.
