* ft <br />A 6nal report for the City of Orono will be presented in a 3-nng binder format which <br />will allow future accessibility infotmation or correspondence to be tnserted directly to <br />provide quick reference for all accessibility infotmation relating to each specific <br />building. <br />Fee Schedule: <br />The proposed fee for the above facilities surveys (Items 1-7 &10) of S3,075. <br />The proposed fee for the Self Evaluation (Items 11-15) is a total of $1,100. <br />Expenses are in addition to the fee and will be billed separately to include: mileage @ <br />.23 per mile, copying at cost, secretarial sendees @ S12 per hour, film and film <br />processing, etc. E.xpenses are not to exceed 5600. <br />The additional fees for item #8 is 5500 for one 1/2 day training session or 5800 for <br />two 3 hour training sessions in one day. <br />Additional fees probable costs (Item ^9) is $500. <br />Contract start date schedule will be upon complete execution of the contract with an <br />expected six week work schedule after the completion of the on-site surveys. <br />If you have any question- 'egarding the proposal or the services available, please <br />contact Julee Quarve-Peterson at (612) 374-3031. <br />Julee Quarve-Peterson <br />President of <br />Julee Quarve-Peterson, Inc. <br />Authorized Signature for <br />The City of Orono <br />Date Date
