A final report for the City ofOrono tvill be presented m a 3-nng binder fonnat which <br />will allow, future accessibility information or correspondence to be rnsetted direaly to <br />provide quick reference for all accessibility information relating to each specific <br />building. <br />Fee Schedule: <br />The proposed fee for the above facilities surveys (Items 1-7 &10) is a tmal of $3,975. <br />The proposed fee for the Self Evaluation (Items 1 1-15) is a total of $1,100. <br />Expenses are in addition to the fee and will be billed separately to include; mileage (ffi <br />.23 per mile, copying at cost, secretarial services @ $12 per hour, film and film <br />processing, etc. E.xpenses are not to exceed $600. <br />The additional fees for item #3 is $500 for one 1.'2 day training session or $800 for <br />two 3 hour training sessions in one day. <br />Additional fees probable costs (Item “9) is $500. <br />Contract stan date schedule will be upon complete execution of the contract with an <br />expected six week work schedule after the completion of the on-site surveys. <br />If you have any questions regarding the proposal or the services available, please <br />contact Julee Quarve-Peterson at (612) 374-3031. <br />Julee Quarve-Peterson, Inc <br />Date <br />Authorized Signature for <br />The City of Orono <br />Date
