This includes the Park Cleanup Event, Tree Planting Program, <br />Spring Cleanup event, and any other educational or outreach event with a <br />Pollution Prevention or Water Quality theme. <br />3. MCM 3: Illicit Discharge Detection and Elimination <br />a) Date(s) and location(s) of IDDE inspections conducted, including any field notes, <br />maps, pictures, videos, and/or test results. <br />b) Reports of alleged illicit discharges received, including date(s) of the report(s), and <br />any follow-up action(s) taken. <br />c) Date(s) of discovery of all illicit discharges. <br />d) Identification of outfalls, or other areas, where illicit discharges have been <br />discovered. <br />e) Sources (including a description and the responsible party) of illicit discharges (if <br />known). <br />f) Labor, materials, and other costs associated with the investigation for invoicing <br />the responsible party. <br />g) Action(s) taken by the City (i.e correspondence, Notice of Violation), including <br />date(s), to address discovered illicit discharges. <br />h) Enforcement actions, including the following: <br />· Name of the person responsible for violating the terms and <br />conditions of the City’s ordinances. <br />· Date(s) and location(s) of the observed violation(s). <br />· Description of the violation(s). <br />· Corrective action(s) (including completion schedule) issued by the <br />City. <br />· Referrals to other regulatory organizations (if any). <br />· Date(s) and proof violation(s) resolved. <br />4. MCM 4: Construction Site Runoff Control <br />a) Site Plan Reviews, including the following: <br />· Project name. <br />· Location. <br />· Total acreage disturbed. <br />· Owner and Operator of proposed construction activity. <br />· Proof of notification to obtain coverage under the CSW Permit or
