DOCUMENTATION AND ASSESSMENT PLAN <br /> Documentation <br />The following information shall be documented: <br />1. MCM1: Education and Outreach <br />a) Activities held, including dates, to reach each target audience, and the estimated <br />audience for each completed activity. <br />b) Quantities and descriptions of educational materials distributed, including dates <br />distributed. This includes newsletters and website hits. <br />c) Staff training for Illicit discharge recognition and reporting. This includes all field <br />staff. <br />d) Staff training for illicit discharge detection and elimination. This includes those <br />responsible for investigating, locating, eliminating illicit discharges, and/or <br />enforcement. <br />e) Staff training for construction site runoff control. This includes those responsible <br />for conducting site plan reviews, site inspections, and/or enforcement. <br />f) Staff training for post-construction stormwater management. This includes those <br />responsible for conducting site plan reviews, site inspections, and/or <br />enforcement. <br />g) Staff training for winter maintenance activities. This includes all staff performing <br />any winter maintenance activity (i.e. calibration and maintenance of equipment, <br />deicing operations, and snow plowing/management). <br />h) Staff training for SWPPP activities. This includes those responsible for municipal <br />operations (i.e. waste storage and disposal, stockpile management, vehicle <br />maintenance, street sweeping, park management, road maintenance, and <br />application of herbicides, pesticides, and fertilizer), operation and maintenance <br />of structural BMPs (i.e. sump manholes, stormwater treatment devices, <br />separators, ponds, outfalls, etc.), pond and outfall assessments, and management <br />of TMDL waste load allocations. <br />i) Staff training documentation shall include: 1) general subject matter covered, 2) <br />names and departments of individuals in attendance, and 3) date of each training <br />event. <br />2. MCM 2: Public Participation/Involvement <br />a) All relevant written input submitted by persons regarding the SWPPP. <br />b) All responses from the City to written input received regarding the SWPPP, <br />including any modifications made to the SWPPP as a result of the written input <br />received. Responses to written input shall be provided to the person originally <br />submitting the comment within three (3) business days, providing information on <br />City procedures and potential modifications to be evaluated. Additional response <br />shall be provided once evaluation of input is complete. <br />c) Date(s), location(s), and estimated number of participants at events held for <br />purposes of providing public opportunity to provide input on the adequacy of the <br />SWPPP.
