Complaint Documentation and Processing <br /> The Permittees shall document all complaints by maintaining a record of all applicable <br /> information concerning the complaint, including the following: <br /> • Name of complainant, address,phone number, and e-mail address. <br /> • Precise property description or parcel number. <br /> I . Name of Permittees representative receiving Complaint and date of receipt. <br /> • Nature of Complaint and the applicable Site Permit conditions(s). <br /> ■ Activities undertaken to resolve the Complaint. <br /> 'I . Final disposition of the Complaint. <br /> The Permittees shall designate an individual to summarize `-omplaints for the Commission. This <br /> person's name,phone number and email address shall accompany a complaint submittals. <br /> A Person presenting the Complaint should to the extent possible, include the following <br /> information in their communications: <br /> • Name, address,phone number, a - ddress. <br /> • Date <br /> • Tract or parcel <br /> ■ Whether the complaint relate oute permitor(2) a compliance issue. <br /> F. Reporting Requih me •, <br /> • The Permittees shall report all co .laints to t;!„-, Com n si ccording to the following <br /> schedule: <br /> Immediate Reports: Alsubstantial complaintssh. be reported to the Commission the same <br /> day received, or onth Howling work day complaints received after working hours. <br /> Such reports are to be directed to High- ransmission Line Permit Compliance, 1-800- <br /> 657-3794, or by e-mail to: DOC.energypermitcompliance@state.mn.us, or voice messages are <br /> ' acceptable. <br /> Monthly Reports: By the 15th of each month, a summary of all complaints, including <br /> 1 substantial complaints received or resolved during the preceding month, shall be Filed to Dr. <br /> Burl W. Haar, Executive Secretary,Public Utilities Commission, using the Minnesota <br /> Department of Commerce eDocket system(see eFiling instructions attached to this permit). <br /> If no Complaints were received during the preceding month,the Permittees shall submit(eFile)a <br /> summary indicating that no complaints were received. <br /> Page 2 of 3 <br /> I <br />
