King; Michael D Olmstead; Melanie Curtis Subject: RE: Nelson Resto(12-0975)-Henn Cty Permit VIOLATION Doug, This is regarding Hennepin County Permit 2014-U1010.We received a complaint that traffic control was not adequate at this site this morning.That was surprising to us since nobody had contacted either our office or our inspector Mike Olmstead that this work was commencing.The following are special provisions from your permit: Special provisions: Contact Mike Olmstead at(612) 221-7162 or via email at michael.olmstead(o)hennepin.us at least 24 hours prior to construction. Failure to contact either Mr. Olmstead or the Permits Office shall be considered a violation of the permit and grounds for dismissing contactor from job site. Daily permission required from Hennepin County Permits Inspector or Office before beginning work between November 15th and April 15th. Temporary traffic control layouts on County roads shall conform to the most recent version of the Minnesota Manual on Uniform Traffic Control Devices. Since none of the 3 special provisions were followed today, I directed Mike Olmstead to have your operations removed from the right-of-way.You must contact Mike Olmstead prior to working in the right-of-way and any day that you are flagging traffic. In addition,a copy of your permit must be on-site at all times when working in the rights-of-way. I am confident that you will follow the conditions of your Hennepin County Permit from this point forward and that future corrective action will not be necessary.
