The on-site sewage t eatment system at the above address has been inspected and appears to fall in o the category checked below. TANK CONDITION (5-10): Tank inspection indicat s: 5 Punpout not need at this time. 6 Soltds accumulati n in tanks indicates they should be punped out this year to help prevent future pro lems. 7 Solids accumulati n in tanks is at a critical level. Tanks should be punped out as soon as possible. 8 System is dischar ing to the surface. Tanks must be pumped out within 48 hours to eliminate surface ischar�e. 9 Inspection risers missing-tanks could not be inspected. Inspection risers (4" dia. pipe) must be ins alled in each tank at nex punpout. If tanks have not been pumped out within the last three years, they sh uld be pu�ed out now. 0 Inspection pipe i located directly over tank baffle (does not give accurate measurement of solids ccunulation). If tanks ha e not been pumped out within the last three years, they should be pixnped out now. DRAINFIELD CONDITION 17-14 : Dr ield inspection i dicates: Drainfield is dry, no surfacing evident. Sane evidence of s rfacing, not critical yet. 13 Drainfield is sat rated and visibly discharging untreated effluent to the surface. Contact the C ty lnspector imnediately. Repairs must be completed within 90 days. 14 Drainfield extent nd condition unknown.
