<br />June 21, 2016 <br /> <br /> <br />City of Orono <br />Attn: Adam Edwards <br />PO Box 66 <br />Orono, MN 55323 <br /> <br />RE: Letter of Agreement <br /> North Well Project Construction Services <br /> Orono, MN <br /> BMI Project Number: C13.111233 <br /> <br />Dear Adam: <br /> <br />This Letter of Agreement is for providing Construction Services associated with the Orono North Well <br />(Well 4) project that was bid on Thursday June 16, 2016. <br /> <br />Scope Clarifications <br />Bolton & Menk will provide all standard construction services associated with the said project including: <br /> <br /> Review of shop drawings <br /> Coordination with the Minnesota Department of Health (MDH) to obtain required permits <br />o Staking nearby parcel boundaries and staking of the well with offset stakes <br />o 1 Meeting at the site with the MDH to approve the staked well location <br /> Project management <br />o Strategic planning and scheduling, managing quantities and payment vouchers, day to day <br />communications <br /> Coordination of subconsultants <br /> Construction observation <br /> 7 meetings at the site <br />o 1 Preconstruction meeting <br />o 3 Progress meetings <br />o 2 Strategic project meetings <br />o 1 Final meeting <br /> Start-up services <br />o Prepare and administer punch list <br />o Conduct startups <br />o Prepare project completion documents for final payment and project closeout. <br />o Develop as-built documents <br /> <br />Budgeted Costs <br />We propose to provide the construction services at our standard hourly rates, a total Not To Exceed cost of <br />$43,925. As noted in the letter recommending award of the project; this cost is based off of Dual Rotary <br />construction method, and costs would be more if the construction contract were awarded to E.H. <br />Renner/Cable Tool construction method.
