ON-SITE SEWAGE TREATMENT SYSTEMS <br />Page 2 <br />3. Description of Site Evaluation Report. Each report shall include <br />a plot plan and supporting data. <br />A. Plot Plan -A scale drawing of the entire lot showing the <br />following: <br />1. All property lines and lot dimensions. <br />2. All existing and proposed structures. <br />3. All existing or proposed well locations, or water <br />supply piping. <br />4. Relative elevations of house, lot corners and <br />drainfield areas. <br />5. Slope or ground at drainfield sites by contour lines or <br />direction arrows and slope percentages. <br />6. Location of all percolation test holes and soil borings <br />with identifying symbols and relative ground elevations <br />of each. <br />7. Primary and alternate drainfield areas identified. A <br />minimum of six (6) soil borings and six (6) percolation <br />tests is generally sufficient to identify primary and <br />alternate sites. <br />8. Distance from primary and alternate drainfield areas to <br />property lines, well locations, and any lake, stream, <br />marsh or drainage channe 1 within 7 5 feet of any part of <br />the system. <br />B. Site Evaluation Report -The supporting data recording the <br />information gathered by on-site soil analysis including: <br />1. Log of all soil borings keyed to the plot plan showing <br />diameter and depth of hole, soil profile and ground <br />water conditions. <br />2. Log of all percolation tests keyed to the plot plan <br />showing diameter and depth of hole, soil profile, <br />percolation rate expressed in minutes per inch and <br />ground water conditions if encountered. <br />3. Description of the testing methods and equipment used. <br />4. Date(s) of testing, name, and MPCA Certificate Number <br />of Site Evaluator. <br />GENERAL SITE REQUIREMENTS <br />1. Site evaluation should identify enough area for a primary and an <br />alternate (future) system on each lot. <br />2. Setbacks: A) Septic tanks from: <br />House= 20' minimum <br />Well= 75' minimum <br />L~ke, Stream or Wetland= 75' minimum* <br />B) Drainfield from: <br />House, driveway, patio, pool, tennis court, <br />other forms of hardcover which potentially <br />drain surface water onto drainfield = 20' <br />minimum <br />*Certain lakeshore areas will have more restrictive setbacks. Please <br />check with the City staff.
