CITY of ORONO <br />Municipal Offices <br />Post Office Box 66 <br />Crysta1Ba~Minnesora55323-0066 <br />ON-SITE SEWAGE TREATMENT SYSTEMS: SITE EVALUATION FOR SUBDIVISION <br />(Applicable to unsewered areas only) <br />At the time of filing an application for preliminary plat <br />approval, the developer is required to provide a site evaluation <br />report confirming that each proposed building site has sufficient <br />suitable soil for on-site sewage treatment. Required submittals are <br />as follows: <br />1. Plot plan which shall be a copy of the preliminary plat drawing <br />showing all existing structures, wells and drainfield systems and <br />all proposed building sites, well locations, primary and <br />alternate drainfield locations. <br />2. Site evaluation report which shall tabulate data from soil <br />borings and percolation tests on each proposed building site of <br />less than five (5) acres. <br />3. Copy of the current compliance inspectio•n report for each <br />existing on-site system within the subdivision boundaries. <br />4. Site Evaluation data showing that an alternate drainfield site <br />exists for each existing on-site system within the subdivision <br />boundaries. <br />5. Where the site evaluation shows slopes in excess of 12 percent, <br />percolation rates slower than 60 minutes per inch, ground water <br />table less than 5 feet deep or soil classification requiring <br />innovative system design, the site evaluation report shall also <br />indicate specific system design specifications as normally <br />required for construction permit applciations. <br />GENERAL REQUIREMENTS <br />1. Site evaluation reports shall be accepted only if signed by a <br />City licensed, MPCA-Certified Site Evaluator. A list of licensed <br />site evaluators is available. <br />2. Evaluation Report. Proposed sites for all on-site sewage <br />treatment system construction or alteration shall be fully <br />explored, evaluated'and reported. The City of Orono shall be <br />notified at least 24 hours prior to any soil boring or <br />percolation testing so that an inspector may be present at the <br />site. The location of all soil borings or percolation tests <br />shall be indicted on the plans and certified by the site <br />evaluator.
