TO: Mayor and City Council <br />ram: Mark Bernhardson, City Administrator <br />DATE: January 31, 1986 <br />SUBJECT: Leave of Absence - Lyle Oman <br />Mr. Oman has made a request for a leave of absence based on his <br />need to undergo extensive foot surgery during the month of <br />February. Given the fact that he will get some compensation from. <br />Worker's Comp (from his previous employer) for the time he is <br />off, he ha elected not to take any sic leave or vacation from <br />the City during this period of time. The request is for a <br />maximum leave of absence of 30 working days which we request the <br />Council approve. In his leave of absence he will have <br />availability of the applicable health insurances at his expense <br />for over 30 days. There is a possibility he will return to work <br />part time prior to the 30 working day leave period. <br />PROPOSED MOTION - Moved by , seconded by , to grant a leave <br />of absence to Mr. Lyle Oman mom February St to March 18, 1986 <br />under the conditions outlined above.
