The Grantee must maintain: <br />1) Written standards of conduct covering conflicts of interest and governing the actions of its <br />employees engaged in the selection, award, and administration of contracts. <br />2) Support documentation of the purchasing and/or bidding process utilized to contract services in <br />their financial records, including support documentation justifying a single/sole source bid, if <br />applicable. <br />3) The Grantee must not contract with vendors who are suspended or debarred in Minnesota <br />(http://www.mmd.admin.state.mn.us/debarredreport.asp). <br />20.Amendments, non-waiver, and assignability <br />a. Any amendment to an award must be in writing and will not be effective until it has been executed <br />and approved by the same parties who executed and approved the original grant award, or their <br />successors in office. <br />b. If State fails to enforce any provision of this Grant Application and Assurances or the Official Grant <br />Award Notice, that failure does not waive the provision or State’s right to enforce it. <br />c. Grantee shall neither assign nor transfer any rights or obligations under this agreement without the <br />prior written consent of State. <br />21.Financial Statements <br />All Non-Governmental Organizations (NGO’s) applying for grants in the state of Minnesota must <br />undergo a financial review prior to a grant award made of twenty-five thousand dollars ($25,000) and <br />higher. <br />In accordance with the Policy on the Financial Review of Nongovernmental Organizations <br />(https://mn.gov/admin/government/grants/policies-statutes-forms/), please submit one of the <br />following documents with your application if you are a Non-Governmental Organization, based on the <br />following criteria: <br />a. Grant applicants with annual income of under fifty thousand dollars ($50,000), or who have not <br />been in existence long enough to have a completed IRS Form 990 or audit should submit their <br />most recent board-reviewed financial statements. <br />b. Grant applicants with total annual revenue of fifty thousand dollars ($50,000) or more and less than <br />seven hundred and fifty thousand dollars ($750,000) should submit their most recent IRS Form <br />990. <br />c. Grant applicants with total annual revenue of over seven hundred and fifty thousand dollars <br />($750,000) should submit their most recent certified financial audit. <br />119
