1 B <br />e.Complete and return application materials. Note that the Division of <br />Licensing must provide written notice to the local zoning authority 2Q <br />days prior to issuance of license. Keep this requirement in mind in <br />your planning. Return the application three months prior to your <br />planned opening date, if possible. <br />f.Contact the licensor for an on-site, prelicensing inspection to verify <br />compliance with all requirements. <br />A program may not open until your application for license is approved <br />and the license has been issued. The license must be conspicurusly <br />displayed on the premises. <br />h.Once licensed, the operator is responsible for timely submission of <br />license renewal forms and requests for changes in the terms of the <br />license. <br />The requirements for licensing are found in Minnesota Rules, parts <br />9503.0005 through 9503.0170, which must be carefully reviewed before <br />proceeding further. <br />A COPY OF THE RULE IS ENCLOSED <br />The licensor whose card is enclosed will provide assistance and should <br />be contacted early in the planning process, preferably three to six <br />months prior to opening. The licensor's telephone number is included on <br />the card Within 90 days of receipt of a completed application, <br />including all necessary approvals, the licensor will recommend issuance <br />or rienial of the license. After the center is opened, the licensor will <br />also be providing supervision through periodic licensing reviews, <br />unscheduled drop-in visits, and investigation of complaints. <br />Day care facilities may not open nor advertise until all licensure <br />requirements have been fulfilled and approved by the Department of Human <br />Services, Division of Licensing. <br />A schedule of uctes and times for upcoming meetings is attached.
