-5*10 E <br />H.documentation of completion of the in service training required by <br />part 9503,0035, subpart 4, showing the training topic, source of <br />training, number of hours completed, and method used to document <br />mastery of the subject; <br />I.documentation of procedures for and completion of an annual <br />evaluation of the staff person's work and specification of <br />inservice training needs; and <br />J. documentation of any disciplinary action including termination. <br />The license holder shall not disclose a staff person's personnel record <br />to any person other than the staff person, the staff person's legal <br />representative, the staff person's supervisor, or employee of the license <br />holder designated by one license holder to maintain personnel records, and the <br />commissioner, unless the staff person has given written consent or as <br />otherwise required by lav. <br />9503.0125 CHIJDREN'S RECORDS. <br />At the time of enrollment In the center, the license holder must ensure <br />that a record is maintained on each child. The record must contain; <br />A. <br />B. <br />C. <br />the child's full name, blrthdate, and c\irrent homo address; <br />the name, address, and telephone nuiiA>er of the child's parent; <br />Instructions on how the parent can be reached when the child Is <br />attending the center; <br />D.the names and telephone mimbers of any persons authorized to take <br />the child from the center; <br />E.the names, addresses, and telephone numbers of the child's source <br />of regular medical and dental care and the source of medical and <br />dental rare to be used in case of an emergency; <br />F.the names, addresses, and telephone numbers of two persons to be <br />contacted If a parent cannot be reached in an emergency or when <br />there is an injury requiring medical attention; <br />G,the heelth form and Immunization Information required by part <br />9503.0140; <br />M.written authorization for the license holder to act in an <br />emergency, or idien a parent cannot be reached or is delayed; <br />I.mrltten authorization, if granted, for the license holder to <br />adainlster Ipecac syrup according to part 9503.0140; <br />J. tho hours and days of the week the child will attend the center;
