213944v6 <br />19.2 Sick leave may be used only for days when the employee would otherwise have been at <br />work. It cannot be used for scheduled days off. Employee must report the reason for the <br />absence within one-half (1/2) hour of the beginning of the work day to their supervisor. <br />Sick leave may be used to care for employee’s immediate family. <br /> <br />19.3 Severance Pay. Each employee with at least three (3) years, but less than twelve (12) years, <br />of service with the employer will receive, upon honorable termination of employment, one- <br />third (1/3) of the employee's accumulated sick leave as severance pay. Each employee with <br />twelve (12) years or more of service with the Employer will receive, upon honorable <br />termination of employment, one-half (1/2) of the employee’s accumulated sick leave as <br />severance pay. These funds shall be deposited into a designated Post-Employment Health <br />Care Savings Plan. <br /> <br />19.4 An employee may donate up to sixteen (16) hours of sick leave per calendar year to a fellow <br />employee. <br /> <br />19.5 Active Worker’s Compensation. Employees receiving Worker’s Compensation benefits <br />because of job connected injury or sickness shall, if he/she assigns his/her check to the <br />City, receive full wages for that period. Or, if any Employee is receiving Worker’s <br />Compensation benefits and does not assign his/her check, he/she will be paid for the <br />difference between the Workers’ compensation allowance and his/her regular weekly pay. <br /> <br />19.6 Long Term Disability Insurance. Employees may enroll in long term disability in the same <br />manner as the benefit is being offered to other City employees. <br /> <br />ARTICLE 20 – LEAVE <br /> <br />20.1 Funeral: All full-time regular employees shall be granted three (3) days paid leave for a <br />death in employee’s immediate family. Means mother, father, spouse, son, daughter, aunt, <br />uncle, niece, nephew, step-child, brother, sister, brother-in-law, sister in-law, mother in- <br />law, father in-law, grandparents and grandchild of the employee or the employee’s spouse. <br /> <br />ARTICLE 21 – INSURANCE <br /> <br />21.1 Effective January 1, 2023, In accordance with the Affordable Care Act, the EMPLOYER’s <br />contribution per employee receiving single medical insurance coverage is $915.00 per <br />month toward a cafeteria benefit plan that offers group health, life, long-term disability and <br />dental insurance, subject to the health insurance opt-out provisions set out in Paragraph <br />21.4. The EMPLOYER’s contribution per employee receiving family (employee plus <br />spouse and/or dependents) medical insurance coverage is $1,715.00 per month toward a <br />cafeteria benefit plan that offers group health, life, long-term disability and dental <br />insurance, subject to the health insurance opt-out provisions set out in Paragraph 21.4. <br /> <br />21.2 Effective January 1, 2024, In accordance with the Affordable Care Act, the EMPLOYER’s <br />contribution per employee receiving single medical insurance coverage is $915.00 per <br />month, (however, at no time shall the Employer contribution be less than 100% of the
