Pronoun references in the Plan shall be <br />deemed to be of any gender relevant to the context, and words used in the singular <br />may also include the plural. <br />Article III. Eligibility and Participation Conditions <br />Section 3.1 Participation Conditions. As a condition to participation and receipt <br />of benefits under this Plan, an Eligible Employee agrees to: <br />(a) Furnish to the Employer the application to participate <br />provided for in Section 3.2 within thirty (30) days of becoming an Eligible <br />Employee; <br />(b) . Designate a portion of his/her Compensation as Pay <br />Conversion Contributions in accordance with the provisions of Section <br />4.2; <br />(c) Observe all rules and regulations implementing this Plan; <br />(d) Consent to inquiries by the Employer with respect to any <br />physician, hospital, or other provider of Medical Care or other services <br />involved in a claim under this Plan; and <br />(e) Submit to the Employer, or such other agent as the <br />Employer may designate, all reports, bills, and other Information which the <br />Employer may reasonably require. <br />Section 3.2 Application to Participate . As a condition of participation, each <br />Eligible Employee shall execute and deliver to the Employer a written and signed <br />application by which the Eligible Employee applies to participate in the Plan, designates <br />the required amount of Compensation for the Plan Year in question as Pay Conversion <br />Contributions as described in Section 4.2, make a benefit election, and supply any <br />other pertinent information that the Employer reasonably requires. <br />Section 3.3 Commencement of Participation .
