Report Required <br />As authorized by Minnesota Statutes, section 349.213, subdivision 1(a), a statutory or home rule city or <br />county has the authority to adopt an ordinance requiring that organizations contribute ten percent of <br />their net profits derived from lawful gambling conducted at premises within the dt/s or county <br />jurisdiction to a fund administered and regulated by the responsible local unit of government without <br />cost to such fund, for disbursement by the responsible local unit of government of the receipts for (i) <br />lawful purposes, or (ii) police, fire, and other emergency or public safety-related services, equipment <br />and training excluding pension obligations. <br />An organization must submit to the local unit of government by January 31, of each calendar year a <br />form that lists all such revenues collected and expenditures for the previous calendar year.__________ <br />Financial Informationt 10% Contribution <br />Fill in the calendar year for which this report is being completed. <br />a. Indicate the beginning of the calendar year fund balance. <br />b. Fill in the total amount collected during the calendar year <br />c. Fill in the total cost of goods sold during the calendar year. <br />d. Fill in the total cost of allowable expenses for the calendar year. <br />e. Add b and c and subtract d to calculate the net profit for the calendar year. <br />f. Add e and a to determine the end of calendar year fund balance. <br />g. Fill in Lawful Purpose Expenditures. This should match the submitted copies of the LG503 <br />state forms. <br />Allowable Expenditures <br />All gambling expenditures from the fund must be for: <br />• Lawful purposes authorized by statute, or <br />• Amounts allocated to a city or county for police, fire, and other emergency or public safety-related <br />services, equipment, and training, excluding pension obligations. <br />The chart below briefly outlines the codes and lawful purposes expenditures (contributions) that are <br />allowed. If you need additional information, refer to Minnesota Statutes, section 349.12, subd. 25. <br />A contribution may not be made to an organization licensed to conduct lawful gambling that <br />contributed to the fund. <br />A-1 To and by 501(c K3) organizations or to and by 601(c X4) festival organizations. <br />A-2 relieving the effects of poverty, homelessness, or physical or mental disability. <br />A-3 Treatment of delayed posttraumatic stress and compulsive gambling. <br />A-4 Funding a public or private nonprofit education institution registered with or <br />accredited by Minnesota or any other state. <br />A-5 A scholarship fund. <br />A-6 Recognition of humanitarian or military service. <br />A-7 For activities and facilities benefiting youth under age 21. <br />A-8 Not applicable. <br />A-9 Not applicable. <br />A-10 Expenditures for police, fire, and other emergency or public safety-related services <br />equipment, ,equipment, and training <br />A-11 To and by a nonprofit organization which is a church or a body of communicants. <br />A-12 Not applicable. <br />A-13 Wildlife management project that benefits the public-at-large provided that the <br />Department of Natural Resources approves it before the expenditure is made. <br />A-14 Costs related of grooming and maintaining snowmobile trails that are grant-in-aid <br />trails, or other trails open to public use, provided that the commissioner of natural resources has <br />approved the expenditure before it is made. <br />A-15 Conducting nutritional programs, food shelves, and congregate dining programs <br />primarily for persons who are age 62 or older or disabled. <br />CertiHcation <br />• Have the person who is responsible for the fund and the preparer of the report sign and date the <br />report. <br />• Include the name and telephone number of a contact person.____________________________ <br />G: \ Kittock\ FOR.MS\ GAMING 10 DOC
