In preparation for submittal of this Application for Payment, several
meetings were held with the contractor to determine the appropriate final payment.
Attached to the Application for Payment is Change Order No. I which documents some e.xtra costs
which were incurred during the course of the project. By way of explanation, if you refer to Item
No. 4 in Change (jrder No. 1 this item documents the additional silt fence necessary to satisfy
concerned parties during construction. The price for the additional silt fence is at the unit price in
the original bid. Item No. 3 m Change Order No. 1 is for covering the surcharge with mulch until
such time as the surcharge can be removed and the project completed. This was added to the project
at a negotiated price with the contractor.
The second item on the Change Order is for additional work associated with the contractor setting
settlement plates on top of the sand building pad prior to placement of the surcharge. The settlement
plates are necessary to monitor the rate of settlement of the sand so that the soil consultant, GME,
can recommend when the building can be constructed on the pad. The pad is anticipated to be ready
for build‘ng construction according to the current schedule for the project. Due to the cold weather,
the settlement plates had to be redesigned by GME before they could be installed by the contractor,
hence the additional cost.
Finally, Item No. I on Change Order No. 1 documents the value of some extra select granular
borrow’ left on the site. In the process of constructing the sand backfill for the building pad, sand
backfill would occasionally fieeze such that the top portion of the sand backfill needed to be
removed prior to placement of additional unfrozen material. The accumulated stock pile ot sand will
have value to the building contractor because the sand could be used as subbase for the entrance
drive and parking areas. Ingram Excavating believes that there is much more than 500 cubic yards
of select granular borrow left on site, but as a concession to the uncertainty as to the exact volume,
the amount of excess select granular borrow left on site has been reduced to 500 cubic yards, wbch
Ingram is guaranteeing will be available. ITie unit price for the select granular borrow has also been
reduced.
Short Biott Htndifcksoa Inc.YaurTntUMRHOuict Bwal Opportimlly Enfiployw
i
