If provided under applicable state <br />law, your continuation and/or conversion rights, and the r ights of those who are covered through you, are <br />described in the separate materials that have been provided to you either directly by the carrier (the <br />insurance company) or by your Employer. If you have not been provided this information, you should <br />contact the Plan Administrator. <br />
