© Hitesman & Wold, P.A. 2019 <br />Cafeteria Plan <br />Summary Description (3-11) <br />22 <br />3.4 What if I am no longer eligible? <br />If you cease to be eligible for coverage under the Group Dental Plan, your coverage under that <br />Plan will terminate in accordance with the terms and conditions of that Plan. I n most cases, if you lose <br />coverage under the Group Dental Plan, your participation in this portion of the Cafeteria Plan will cease as <br />well, subject to the change in election rules described in Section 1.8. <br /> <br />If you cease to be eligible to participate in this Cafeteria Plan, your ability to pay for coverage under <br />the Group Dental Plan on a pre-tax basis through this portion of the Cafeteria Plan stops. <br />3.5 Can coverage be continued? <br />If you cease to be eligible for coverage under the Group Dental Plan, you and any others who <br />receive their coverage through you may be able to continue that coverage. Continuation coverage is <br />available in accordance with the Consolidated Omnibus Budget Reconciliation Act of 1985 ("COBRA”), the <br />Uniformed Services Employment and Reemployment Rights Act of 1994 ("USERRA"), and applicable <br />continuation requirements under state law. These continuation rights are described later in this summary. <br />3.6 What if I am subject to a medical child support order? <br />The Group Dental Plan recognizes child support orders regarding the provision of medical coverage <br />for a child, including orders under the Child Support Performance and Incentive Act of 1998 to the extent <br />required by law. If a child is enrolled in the Group Dental Plan pursuant to a chi ld support order, you will <br />be able to pay the cost of that coverage through this portion of the Cafeteria Plan, provided you are eligible <br />to participate as described above.
