© Hitesman & Wold, P.A. 2019 <br />Cafeteria Plan <br />Summary Description (3-11) <br />2 <br />PART I. <br />GENERAL INFORMATION ABOUT THE PLAN <br />1.1 What is the purpose of the Cafeteria Plan? <br />The purpose of the Cafeteria Plan is to allow eligible employees to use funds provided through <br />employee salary reduction and Employer flexs to choose (and pay for) certain benefits made available by <br />the Employer. <br />1.2 When did the Cafeteria Plan take effect? <br />This Plan became effective September 1, 2022. <br />The Plan operates on a Plan Year running from January through December. <br />The Plan Year beginning on September 1, 2022 is a short Plan Year that ends on December 31, <br />2022. <br />1.3 What Optional Benefits are offered through the Cafeteria Plan? <br />This Plan makes the following checked “Optional Benefits” available: <br />GROUP PREMIUMS: <br /> Group Medical Benefits <br />The group medical benefit allows a Participant to pay the employee's share of the cost for <br />medical coverage made available by the Employer with pre -tax dollars through: <br /> Salary reduction Employer contributions <br /> Group Dental Benefits <br />The group dental benefit allows a Participant to pay the employee's share of the cost for dental <br />coverage made available by the Employer with pre-tax dollars through: <br /> Salary reduction Employer contributions <br /> <br />OTHER: <br /> <br /> Dependent Care Flexible Spending Account <br />The dependent care reimbursement benefit allows a Participant to fund an account with pre -tax <br />dollars through: <br /> Salary reduction Employer contributions, that may be used to reimburse the Participant for <br />eligible dependent care expenses. <br /> Health Flexible Spending Account <br />The medical expense reimbursement benefit allows a Participant to fund an account with pre -tax <br />dollars through: <br /> Salary reduction Employer contributions, which may be used to reimburse the Participant <br />for eligible medical expenses. <br /> Limited Scope Health Flexible Spending Account <br />The limited scope medical expense reimbursement benefit allows a Participant to fund an account <br />with pre-tax dollars through:
