5.2.1.5.2.1 MCES Affidavit-B—Affidavit of Business Use— Manufacturing <br /> This form is required to be submitted with the other items listed on the SAC Determination <br /> Application if the business use is a manufacturing establishment. This affidavit must be filled out and <br /> signed by the business owner. <br /> 5.2.1.5.2.1 MCES Affidavit-C— Property Use <br /> This form is required when requested by MCES to validate the use of a property when no SAC <br /> history or documentation exists. This form must be filled out and signed by the customer community <br /> building official and the building official's manager. <br /> 5.2.1.5.2.1 MCES Reclaim-C— Reclaim (Conveyor System) <br /> A submittal checklist for reclaim conveyor car wash systems, all of the items on this checklist must <br /> be submitted with the SAC Determination Application. <br /> 5.2.1.5.2.1 MCES Reclaim-R— Reclaim (Rollover System) <br /> A submittal checklist for reclaim rollover car wash systems, all of the items on this checklist must be <br /> submitted with the SAC Determination Application. <br /> 5.2.1.5.3Determination Letters <br /> MCES will complete Determinations for Commercial Properties upon request of the Community. <br /> MCES prefers,and it is likely quicker, if the Community staff complete straight-forward SAC <br /> Determinations using the SAC criteria as found in Appendix A.When MCES completes a <br /> Determination,a letter will be sent electronically to the Community and copied to the applicant that <br /> states the Net SAC. The Determination Letter will include the name and location of the project, SAC <br /> charge and Credit calculations and the Net SAC. Each Determination expires two years from the date <br /> of the letter.If the Community permit has not been issued before the Determination Letter expiration <br /> date,the Determination Letter is void.Also, if the building plans change in any material way before <br /> the Community permit has been issued,the Determination Letter is void and a new Determination <br /> must be completed. In all cases,the Community is responsible for verifying accuracy of the <br /> Determination at final inspection to ensure the proper number of SAC units have been charged for <br /> each business and/or property. <br /> 5.2.1.5.4End of Calendar Year Determinations <br /> To pay the current year's SAC rate,all Determination requests for MCES must be submitted with <br /> complete documentation no later than 10 MCES operational days before the end of the calendar year <br /> so MCES has time to complete the Determination.The Community permit or license must be issued <br /> no later than December 31. (i.e.to pay the 2015 SAC rate, all Determination requests must be <br /> submitted no later than December 18,2015 and the Community permit or license must be issued no <br /> later than December 31,2015.) <br /> 5.2.1.5.5 Exclusions from Determination Requirements <br /> If a property is obtaining a cosmetic remodeling permit,where there is no change of use or size,no <br /> SAC Determination is necessary and no SAC will occur. If a property is changing size but not use, <br /> additional SAC will be required on the incremental Capacity Demand. <br /> Other examples of projects that do not need a SAC Determination are: <br /> • buildings originally determined as 100%office and remodels space of less than 2,600 square <br /> feet to a meeting room,no Determination is necessary but the charge but the change of use <br /> should be reported with the monthly SAC Activity Report on the SAC-C form; <br /> Page 118 <br />
