Big Island Nature Park Special Activity Permit Application (continuec� � <br /> Submittals <br /> Please give consideration to the following list of documents and/or permits which may be required prior to City <br /> and Council approval. <br /> The application rriust be submitted at least 14 days prior to the event. <br /> Documents <br /> ❑ Site plan identifying the locations within the Park in which the activity will be held. <br /> ❑ Certificate of Insurance Coverage may be required depending on nature of activity. <br /> ❑ For activities lasting more than 3 hours, description and location of sanitary facilities and an emergency <br /> evacuation plan. <br /> ❑ Proof of possession of any license or permit which, under federal, state or local laws or regulations,the <br /> applicant is required to have in order to conduct the special activity, or which, under any such law or <br /> regulation, would exempt the applicant from the licensing requirements of Orono City Code Sec. 22-103(c). <br /> Licenses/Permits (if applicable) <br /> ❑ DNR ❑ Fire Department(Fire Marshal) <br /> ❑ Hennepin County Sheriff's Water Patrol ❑ Health Department(County) <br /> ❑ Lake Minnetonka Conservation District(LMCD) ❑ Temporary Sign <br /> ❑ Building Permit(temporary structure, i.e. tent) ❑ Other <br /> Some or all of the information that you are asked to provide on the application is classified by State law <br /> as either private or confidentiaL Private data is information which generally cannot be given to the <br /> public but can be given to the subject of the data. Confidential data is information which generally <br /> cannot be given to either the public or the subject of the data. Our purpose and intended use of this <br /> information will be used to determine your qualification for the permit or license requested. If you refuse <br /> to supply the information,the permit or license may not be issued. <br /> Please answer the following questions related to this special activity permit application: <br /> PARTICIPANTS/GUESTS <br /> Approximately how many people are expected at the activity? <br /> Is this activity open to the public? ❑ Yes ❑ No <br /> DOCKAGE <br /> Approximately how many watercraft are expected? <br /> How many of these will be tied up at the Park docks at one time? <br /> FEES BEING CHARGED <br /> Will the activity have any fees collected? ❑ Yes ❑ No <br /> Will there be any tickets sold for the activity? ❑ Yes ❑ No <br /> What is the purpose for any money that is collected? <br /> LARGE ACTIVITY <br /> Will there be more than 200 participants? ❑ Yes ❑ No If yes, please provide a site map of the location of <br /> activities, sanitary facilities provided by applicant, garbage containment, etc. Must meet requirements of <br /> Orono Code Sec.38-802. <br /> Will any special services be required from the Police Department or any other City department? ❑ Yes ❑No <br /> Describe the required services: <br /> ADDITIONAL INFORMATION <br /> Depending on the nature of the activity,additional information may be required. <br />
