Special Event Permit Application (continued) <br />Snbnuttals-. <br />Please give consideration to the following list of documents and/or permits which may be required prior <br />to City and Council approval. The application should be submitted at least 14 days prior to the event. <br />Documents <br />[Z Site plan identifying buildings, driveways, local streets, parking locations, temporary structures (include <br />size), temporary restrooms, or any cordoned off area(s) for special events <br />❑ Proposed parade route or location of event <br />❑ - Signature of the owner of the property on which the special event is to be held. <br />❑ Approval of Homeowners Association if the event affects private property where a Homeowners <br />Association has been established. <br />❑ Proof of written notice to all property owners within 350 feet of the location on which a special event is to <br />be held at least 10 days prior to the event. The notice must contain the date, time, and location of the <br />special event. <br />❑ Shuttle bus service will be required for all Parade of Homes/Home Tours unless applicant demonstrates <br />sufficient on-site parking is available. Shuttle bus plan to be included with application. <br />❑ Proof of possession of any license or permit which, under federal, state or local laws or regulations, the <br />applicant is required to have in order to conduct the parade or special event, or which, under any such law <br />or regulation, would exempt the applicant from the licensing requirements of Orono City Code Sec. 66- <br />223. <br />❑ A policy or certificate of insurance on which the city, its agents and employees, are additional insureds, <br />providing public liability insurance. providing umbrella or all perils coverage in the amount of <br />$12000,000.00. <br />Licenses/Permits Lif applicable) <br />❑ DNR ❑ Lake Minnetonka Conservation District (LMCD) <br />❑ Fireworks ❑ Peddler, Solicitor or Transient Merchant <br />❑ Fire Department (Fire Marshal) ❑ Temporary Sign <br />❑ Gambling m Building Permit (temporary structure, i.e. tent) <br />❑ Hennepin County Sheriff's Water Patrol ❑ Vendor License <br />❑ Health Department (County) ❑ Other <br />-❑ Temporary Liquor License (State and Local) — Any time alcohol is served or sold to the general public, <br />or if alcohol is sold at a private partylevent-even if included in ticket price.
