Reconciliation Act) can participate at the same premium rate as active Employees until age 65. The City will <br />not contribute any money towards these benefits. <br />Coverages and levels of benefits provided to retirees under age 65 and their dependents must be <br />identical to that provided for active Employees and their dependents. <br />Retirees age 65 and over may stay in the group indefinitely, but premium rates do not have to be <br />pooled with the active Employee rates and retiree coverage does not have to be identical to active coverage. <br />The retiree must pay the entire premium. <br />Spouse and/or dependent coverage can be continued after retirement only if the Employee carried <br />dependent coverage prior to retirement. Employees are not required to continue spouse and/or dependent <br />coverage after retirement. <br />9.08 Eligibility: <br />9.081 Medical and Dental Insurance: To be eligible for the full dollar Employer contribution for single <br />insurance, an Employee must be regularly employed at least 1560 hours per year. <br />ARTICLE 10 <br />LEAVES OF ABSENCE <br />10.01 Sick Leave: <br />10.011 Accrual - 12 -Month Employees: Sick leave with pay shall be granted to all probationary and <br />regular Employees at the rate of one (1), eight (8) hour working day for each calendar month of full-time <br />service. For this purpose, one (1) calendar month shall be any month in which an Employee is paid for <br />twelve (12) or more working days (ninety-six [96] working hours). Regular part-time employees over 20 <br />hours per week receive a prorated accrual based on hours worked relative to full-time hours worked. <br />Seasonal/temporary Employees are not eligible for sick leave accrual benefits. <br />Employees will not be allowed to accrue more than 960 hours (120 days) of sick leave. Once an <br />Employee accrues 960 hours they will not accrue additional time unless the number of hours drops <br />below 960. <br />10.012 Reporting: Each Employee shall be informed of their unused accrued sick leave on their <br />paycheck. <br />10.013 Application: Sick leave shall be deducted from the accrued sick leave days earned by an <br />Employee. If a holiday falls within the time an Employee is on paid sick leave, that day shall not be <br />counted against accumulated sick leave. Sick leave pay shall be approved only upon submission of a <br />request form. Such form shall be readily available at each Employee location. <br />10.014 Death of Employee's or Spouse's Family: All full-time regular Employees will be allowed up <br />to 3 days paid funeral leave, if requested. This applies for the death of any immediate family, meaning: <br />mother, father, spouse, child, aunt, uncle, niece, nephew, step -child, siblings, in-laws, and <br />grandparents and grandchild. This time will not be deducted from any accruals of the Employee. <br />10.015 Sick Leave of Employee's or Spouse's Family and Illness: Pursuant to Minn. Stat. <br />181.9413, eligible Employees may use up to 160 hours of sick leave in any 12 -month calendar period <br />for absences due to an illness or injury to the Employee's adult child, spouse, sibling, parent, <br />grandparent, stepparent, parent -in-laws (mothers-in-law and fathers-in-law) and grandchildren <br />(biological, adopted or foster grandchildren). This leave is inclusive of, not in addition to, the FMLA <br />leave described in section 10.06. <br />6 <br />196142v2 <br />
