Change of beneficiary. <br />d. Change in type of coverage. <br />Change of marital status. <br />f. Change in number of dependents. <br />Worker's Compensation <br />In accordance with the laws of the State of Minnesota, the City provides coverage for <br />medical expenses in the event of work related injury or disease, plus partial salary continuation <br />in the event of disability, and additional benefits if the injury or disease causes partial or <br />permanent disability or death. <br />Employees are automatically covered under the workers' compensation plan as soon as <br />employment begins. To be eligible for workers' compensation, the injury or disease must arise <br />out of and occur during the course of employment. <br />The City pays the entire cost of this protection. Benefits paid depend entirely on the <br />benefit schedule set up by the State of Minnesota and applicable at the time the work related <br />injury or disease is incurred. <br />If an employee has a work-related injury or disease and is under a doctor's care, that <br />employee cannot change doctors for treatment of this work-related injury or disease without first <br />contacting the City's workers' compensation company and obtaining approval to make this <br />change. <br />Steel Toe Boots Reimbursement <br />Public Works employees and the Golf Course Superintendent shall receive an annual <br />reimbursement as established by resolution for the purchase of steel toe boots. The employee <br />must submit the standard reimbursement form along with a receipt for the purchase. The <br />reimbursement shall not exceed the annual amount established by City Council resolution. <br />Employee Recognition <br />The City recognizes that a formal employee recognition program is an important <br />component of the City's benefit package and helps attract and retain quality employees.
