Defendant violated his conditions of <br /> sentencing and probation violation documents were filed on May 10, 2011. A probation violation hearing was <br /> set for June 10, 2011 and then a Revocation hearing was set for July 29, 2011. The defendant failed to appear at <br /> the July 29, 2011 hearing and a Bench Warrant was issued.
