Vcrification of good-faith shall include a specific response to each of the following <br /> applicable categories. A bidder may include any additional information the bidder believes may be relevant. <br /> 1. Verificatian that the bidder rejected SBE's because they did not submit the lowest bid or they were not � <br /> qualified. Such verification shall include a verified statement of the amounts of all bids received from <br /> potential subcontractors on the project and that the bidder rejected SBE's because they did not submit � <br /> the lowest bid from among such bids or they were not qualified. For each SBE found to be not <br /> qualified,the vezification shall include a statement giving the bidder's reasons for its conclusion. <br /> 2. Verification of effotts to contract by timely written or telephone notice to all appropriate SBE's within , <br /> � identified subcontracting categories listed in the most current, approved certification Directory. If <br /> ; bidders have purchased project plans seven(7)calendar days or fewer before bids are due,documented <br /> a phone calls in lieu of written notice may be permitted. � <br /> � 3. Verification of effo�ts to subcontract, consistent with industry practice, with the SBE's whom the <br /> : bidder has contacted, or who have contacted the bidder. Include the names, addresses, and telephone , <br /> + numbers of a11 SBE's contacted; a description of efforts made to subcontract; and a descriptian of the <br /> � information provided to the SBE regarding the plans and specifications for the portion of the work to <br /> � be performed by subcontractors. If attempts to subcontract actually occurred, provide the dates and � <br /> places of such process and a description of the outcome. <br /> ' 4. Verification that the bidder attempted to recruit SBE's from at least the same geographic azea from � <br /> which it attempted to recruit other subcontractors. <br /> 5. Verification that, consistent with indus�y practice, the bidder gave SBE's necessary access to and � <br /> adequate tune to review all necessary project plans, drawings, specifications and other documents, as <br /> well as adequate timc to prepare subcontract bids. <br /> 6. Verification that, cansistent with industry practice and t1�bidde,r's past practices on similaz projects, � <br /> the bidder selected portions of the work to be performed by SBE's in order to achieve the project <br /> utilization levels,Including consideration of structuring the conhact into economically feasible units to <br /> facilitate meaningful SBE utilization as subcontractors or suppliers. � <br /> � 7. Stateme�t giving the reasons why the bidder and each SBE contacted or each SBE that contacted the <br /> bidder did not succced in reaching a subcontracting agreement. � <br /> ; <br /> � 8.
