Verification of good-faith efforts shall include a specific response to each of the following applicable categories. A bidder may include any additiona! informativn the bidder believes may be relevant. 1. Verification tha#the bidder rejected SeE',s because they did not submit the lowest bid or they were not quaiifed. Such verification shall include a verified sta#ement of the amounts of all bids received from potential subcon#ractors on the project and that the bidder rejecEed SBE's because they did not submit the lowest bid from amortg such bids or they were not qualified. For each S�E found to be not qual'rfied, the verification shall iriclude a statement giving the bidder's reasons for its conclusion. 2. Verification of efforts to contract by timely written or telephone notice to all appropriate SBE's within identified subcontracting categories listed in the most current, approved certification Directory. !f bidders have purchased pr�ject plans seven (7) calsndar days ar fewer before bids are due, dvcumented phone calfs in lieu of written notice may be permitted. 3. Verification of efForts tQ subcontract, consistent with industry practice, with the SBE's whom the bidder has contaCted, or who have contacted the bidder. Include the names, addresses, and telephone numbets of all SBE's contacted; a description ofi efforts made to subcontract; and a description af the information provided to the SBE regarding the plans and specifications for the portion of the work to bs perFormed by subcontractors. If attempts to subcontract actually occurred, provide the dates and places of such process and a description of the outcome. 4. Verification that the bidder attempted to recruit S�E's from at Ieast the same geographic area from which it attempted to recruit other subcontractors. 5. Verification that, consistent with industry practice, the bidder gave SBE's necessary access to and adequate time to review all necessary project plans, drawings, specifications and other documents, as well as adequate time to prepare subconUact bids. 6. Verification that, consistent with industry practice, th�'�bidder's past practices on similar projects, the bidder selected portions of the work to be pertarmed by SBE's in order io achieve the project utilization levels. This Includes considerativn of structuring the contract into economically feasible units to facilitate meaningful SBE utilization as subcontractors or suppliers. 7. Statement giving the reasons why the bidder and each SB� contacted or each SBE that contacted the bidder did not succeed in reaching a subcontracting agreement. 8.
