MNG 790000 <br /> As you may be aware, the general permit authorizing the discharge of treated contaminated <br /> ground water shall expire on February 29, 1996. In order to continue the permitted activity <br /> beyond the expiration date of the general permit, you must submit a completed application for <br /> permit reissuance before the expiration of the existing permit. <br /> Please complete the enclosed application and submit the original to the Minnesota Pollution <br /> Control Agency (MPCA) at the following address by December 31, 1995. <br /> Minnesota Pollution Control Agency <br /> Water Quality Division <br /> Attn: Point Source Compliance Section <br /> 520 Lafayette Road <br /> St. Paul, Minnesota 55155 <br /> The permit application will not be processed unless it has been correctly signed and the required <br /> $85.00 application fee has been submitted with the application. According to Minn. R. <br /> 7001.0060, the application should be signed as follows: <br /> For a corporation, by a principal executive officer. <br /> For a public facility, by a principal executive officer or ranking official (for a city the <br /> mayor must sign). <br /> For a partnership or sole proprietorship, by a general partner or the proprietor, <br /> respectively. <br /> Application for permit reissuance is not a function which can be delegated.
