Sec.13.04 RIGHTS OF SUBJECTS OF DATA
Subd.1. Type of data. The rights of individual on whom the data is stored or to be stored shall be as set forth in this section.
Subd.2. Information required to be given individual.An individual asked to supply private or wnfidential data conceming himself shall be
informed of: (a)the purpose and intended use ofthe requested data within the collecting state agency,political subdivision,or statewide system;(b)
whether he may refuse or is legally required to supply the requested data;(c)any known consequence arising from his supplying or refusing to supply
private or confidential data;and(d)the identity of other persons or entities authorizsd by state or federal law to receive the data.This requirement shall
not apply when an individual is asked to supply investigative data,pursuant to section 13.82,subdivision 5,to a law enforcement officer.
The commissioner of revenue m�y�lace the notice reguired under this subdivision in the individual income tax or roue t�refund
instructions instead of on those forms.
Subd.3.Access to data by individual.Upon request to a responsible authority,an individual shall be informed whether he is the subject of
stored data on individuals,and whether it is classified as public,private or confidential. Upon his further request,an individual who is the subject of
stored private or public data on individuals shall be shown the data without any charge to him and,if he desires,shall be informed of the content and
meaning of that data. After an individual has been shown the private data and informed of its meaning,the data need not be disclosed to him for six
months thereafter unless a dispute or action pursuant to this section is pending or additional data on the individual has been collected or created. The
responsible authority shall provide copies of the private or public data upon request by the individual subject of the data. The responsible authority
may require the requesting person to pay the actual costs of making,certifying,and compiling the copies.
T'he responsible authority shall comply immediately,ifpossible,with any request made pursuant to this subdivision,or within five days of
the date of the request,excluding Saturdays,Sundays and legal holidays,if immediate compliance is not possible.If he cannot comply with the request
within that time,he shall so inform the individual,and may have an additional five days within which to comply with the request,excluding Saturdays,
Sundays and legal holidays.
Subd.4. Procedure when data is not accurate or complete.An individual may contest the accuracy or completeness of public or private data
concerning himself.To exercise this right,an individual shall notify in writing the responsible authority describing the nature of the disagreement.The
responsible authority shall within 30 days either. (a)correct the data found to be inaccurate or incomplete and attempt to notify past recipients of
inaccurate or incomplete data,including recipients named by the individual;or(b)notify the individual that he believes the data to be correct. Data in
dispute shall be disclosed only if the individual's statement of disagreement is included with the disclosed data.
The determination of the responsible authoriry may be appealed pursuant to the provisions of the administrative procedwe act relating to
contested cases.

and other calculations submitted with the permit applicarion. 'The proposed building has been designed to meet the 2000 Minnesota
Energy Code requirements in RES checkVersion 3.5 Release 1 (formerly MECchecl�and to comply with the mandatory
requirements listed in the RES checkInspection Checklist.

DESCRIPTION OF PROJECT:
Second floor master suite addition and garage.
General Conditions
Note: Specifications that are stated in the SCOPE OF WORK may supersede the specifications as
written in General Conditions.
1. Separate writings/drawings constitute a part of this Proposal and are incorporated into this Proposal
by this reference: Matt and Kathy Dolliff A1-A5, X1, and S1 dated 01/29/07 by SLUJPH. Should any
inconsistency exist or arise between a provision of this Proposal and any attached writing or drawing,
the provision of the Proposal shall prevail. If drawings are included with this document, a signature
will be required on this Proposal and on Authentic Construction's copy of those drawings to indicate
that they have been reviewed and approved.
2. Abbreviations Glossarv. Although we have omitted the majority of construction term abbreviations in
this proposal, we continue to make use of a few common construction abbreviations. They are as
follows:
1. PSY: per square yard
2. OC: on center[spacing]
3. SF: square feet
4. PSF: per square foot
5. OSB: oriented strand board
3. All permits as required.
4. Items to be added, deleted, or any clarifications will be handled with an Additional Work Authorization
(AWA). See paragraph 2 of Important Notice to Customer below.
5. Owner to remove all personal items as necessary to perform the work from each area scheduled for
renovations prior to commencement of work.
1. Areas and items specified to be demolished will not be salvaged unless previously agreed
upon and noted herein.
2. In the event that there are items to be saved, they shall be clearly marked prior to the first day
of work and discussed at the preconstruction walkthrough.
6. Engineering for this project will be provided by the supplier/manufacturer of the wood framing
materials, such as the lumber yard, unless specifically stated otherwise.
7. Dumpster/landfill charges included. All debris will be disposed of off site unless otherwise stated
below. See paragraph 9 & 12 of Important Notice to Customer below.
8. A portable toilet facility will be on site during construction.
9. Note: Electrical, if described below, is the electrical as it will be at completion of project and may
include existing circuits and devices.
10. Windows will be Marvin wood ultimate double hung, IowE II glass with argon gas, 6 9/16 jambs, clad
exterior.
11. Doors listed below are maple 2 panel unless otherwise stated below. Doors will be bored for
standard lockset and deadbolt; mortise or other special bore will be an additional charge and will be
handled with an AWA.
12. Door hardware shall be installed at each new interior door and be Provided with a sum total
allowance of$ 100.00 for all said doors.
13. Install drywall is to install '/z" on the walls, taped, sanded, and prepared for paint and 5/8" on the
ceiling, taped, sanded, fogged, and prepared for primer with a skim coating.
14. Cabinetry spec�cations are as follows uniess noted othen�vise:
1. Refer to Interior Room Schedule of Work for room-by-room description of cabinetry, including
species, style, etc.
2. Drawers shall be dovetail hardwood construction with Blum Tandem full extension rails on
Blumotion undermount slides.
3. Blum 135°fully adjustable hinges
4. Stain color of the cabinets will be selected from fabricator's standard colors. Other colors are
available at an additional cost.
5. Any selection of handles/knobs/pulls from Amerock is included for every operating door and
drawer, style and finish to be selected. Other handles are available at an additional cost.
15. The trim (casing, baseboard, crown or other moldings, etc.) is to match the existing as close as
possible using readily available local stock. See paragraph 18 of Important Notice to Customer
below.
1. Baseboard: paint grade F 232 '/2" x 3 '/4"
2. Casing : paint grade F112 with 5/4 stool and F112 apron
3. Crown Molding: Selected from the cabinet makers standard collection [Unless noted
otherwise, new crown moldings shall be installed only at ceiling to cabinet intersections.]
16. Ceramic tile—materials are standard ceramic tile and installation is standard installation. Any
change from ceramic tile to other tile materials such as natural stone or porcelain may increase the
cost. Any change from standard installation to another design such as diagonal, diamond, clipped
corner, hexagonal, etc. may also increase the cost. Also, any sealing or enhancement of tile, stone
or other product, unless included below, will be an additional charge if requested by the owner. Any
change in material, installation pattern, or material costs above or below the allowance will be.
handled with an AWA.
17.All painting is to be supervised by Authentic Construction but paid directly by the homeowner to the
subcontractor. At this point we have 3 different painting bids which are for only the items in this base
proposal (no options).

3. Fillmore Associates, Inc. $16,470.00
The owner sha/l contract direct/y with the painting subcontractor of their choice and the cost for
this work is not included in this proposal.
18. Work area shall be sealed off from the rest of the house best as possible and kept neat and clean on
a daily basis.
19. Up to 16 hours of professional cleaning upon completion of the project is included. Unless
specifically noted otherwise, the cleaning of the exterior of new or existing windows is not included in
the proposal.
SCOPE OF WORK:
Structural
General Demolition & Site Preparations:
1. Exterior: Demo of the roof down to ceiling joists in the affected areas, removal of affected areas of
the garage including ceiling joists, walls, floors, and electrical (saving the existing garage door,
opener, and track for re-use) exterior siding to be removed in affected areas also.
2. Interior: Demo of the closet at the existing first floor stairs, ceiling over the dining area to
accommodate the barrel vault, the entire existing mudroom gutted down to the bare studs (salvaging
the existing lockers for re-install in a new location of the mudroom), and possibly some miscellaneous
work in the kitchen (ceiling) and lower level to allow new mechanical chases
3. General: Demo of inechanicals as required to perForm the new work. Floor protection to be put down
in all areas of the home with construction traffic. Install poly walls with zipper doors between areas
not affected by construction to help control dust in the home.
Foundation Details
1. Excavate for new construction foundation work as noted on drawings including frost footings and post
footings
a. Landscape repairs such as tree removal, final grading and black dirt, sod, seeding, shrubs,
plants, trees, and non-living elements such as retaining walls, fencing (repair, new),
sidewalks, patios, decks, etc. shall be provided by the owner unless specifically mentioned
otherwise.
2. Concrete 8� Masonry
a. Pour concrete footings (sized per plans) and lay block work (sizes and coursings per plans)
with horizontal and vertical reinforcement bars. Pour concrete floors as noted on plans.
3. Backfill and rough-grade site at completion of concrete work.
4. Final grade, black dirt and landscape repairs are by owner (see section 16 of/mportant Notice to
Customer).
Exterior Enveloqe
1. Install floor framing with engineered floor trusses] spaced per manufacturers recommendations with
g/." tongue 8�groove plywood sub flooring, glued and mechanically fastened.
2. Install exterior wall framing using 2x6 SPF, studs spaced 16" OC at the 2"d floor addition, 2x4 SPF
studs at the garage exterior walls

b. Install '/° O.S.B. sheathing on exterior walls
c. Beams as noted on plans
3. Install interior wall framing using 2x4 SPF, studs spaced 16" OC

a. Install Ice and water shield on eaves, up the roof approximately 6' and up the valleys 3' wide.
b. Install 15#felt underlayment for the shingles on the remainder of the roof sheathing.
c. Install galvanized "W° valley and step flashing metal.
d. Install plumbing vents and roof venting as required.
e. Provide and install EPDM roofing on a tapered foam base with metal caps and flashings as
noted at flat roof area.

7. Insulation
a. Walls: fumish and install R-19 batt insulation in the new and remodeled exterior walls with 4
mil poly vapor barrier.
b. Ceilings: furnish and install R-38 batt or blown in insulation with air chutes where necessary
with 4 mil poly vapor barrier.
c. Rims: spray 2 1/2" of closed cell comfort foam to rim areas of addition
d. Foam seal all bypasses and tape all poly seams.
e. Sound Walls: furnish and install R-11 batt insulation in the surrounding bathroom walls for
sound attenuation.
8. Soffit and Fascia
a. Provide and install fascia and soffit (venting as necessary) at new locations to match the
existing as close as possible using readily available materials.
9. Exterior Siding System
a. Install Tyvek (or equal) house wrap installed per manufacturer's recommendations
appropriate for siding system
b. Provide and install cedar, corner boards, ribbons, window wraps, and other exterior trim in
locations and sizes as noted on drawings
c. Provide and install cedar shingle and vertical board and batten siding at new and existing
exterior walls as noted on drawings
d. Apply and install lath coat and standard Cultured Stone Veneer(or equal) masonry veneer at
locations as noted on drawings
10. Exterior Finishes provided by owners painter
11. Wrought Iron
a. Fabricate and install custom wrought iron railing, powder-coat finished with mounting brackets
at the master balcony.

1. Porch renovations
Shore up and remove perimeter walls including existing windows and doors from the existing porch,
frame in for new Marvin windows ( 2 CCM 3260-2W, 1 CCM 4028-2W, 1 CCM 3260-3W) and 2 32°
exterior fullview steel doors with a fireplace in the comer with a stone face, electrical rewired up to
code, T&G knotty pine walls, installed flat stock pine millwork, and insulate the floors, walls, and
ceilings, stain finishing of walls and trim included. Exterior siding and painting included as well at new
walls (above and below new windows).
2. Living room renovations
Remove the existing doors in the living room and replace with a back wall of 9 Marvin windows CCM
3672 operable windows, including sheetrock repairs, framing, re-painting the living room, siding
repairs and painting/enameling.
3. Deck option
Demo the entire deck elements outside the living room and current master bedroom and install a new
band board at removed deck connection point to house.
4. Arched window
Add a triple-wide custom Marvin arched window above the doors to the balcony at the new master
bedroom.
5. Resurface existing deck
At the existing deck outside the porch, demolish the existing railings and remove the deck boards to
expose the existing treated wood joist system. Install new standard color 5/4x6 Trex deck boards
