Sec.13.04 RIGHTS OF SUBdECTS OF DATA �
Subd. 1. Type of data. The righu of individual on whom the data is stored or to be stored shall be as set forth in this section.
Subd.2. Information required to be given individual. An individual asked to supply private or confidential data concerning himself
shall be informed of: (a)the purpose and intended use of the requested data within the collecting state agency,polidcal subdivision,or statewide
system;(b)whether he may refuse or is legally required to supply the requested data;(c)any known consequence arising from his supplying or
refusing ro supply private or confidential data;and(d)the identiry of other persons ot entities authorized by state or federal law to receive the data.
This requirement shall not apply when an individual is asked to supply investigative data, pursuant to secdon 13.82, subdivision 5, to a law
enforcement o�cer.

Subd. 3. Access to data by individual. Upon request to a responsible authority,an individual shall be informed whether he is the
subject of stored data on individuals,and whether it is classified as public,private or confidential. Upon his further request,an individual who
is the subject of stored private or�rublic data on individuals shall be shown the data without any charge to him az►d,if he desires,shall be informed
of the content and meaning of that data. After an individual has been shown the private dara and informed of its meaning,the data need not be
disclosed to him for six mocuhc thereafter unless a dispute or ac6on}xirsuant to this section is pending or additional data on the individual has been
collected or created. The responsible authority shall pmvide copies of the private or public dara upon request by the individual subject of the data.
The responsible authority may require the requesdng person to pay the acmal costs of maldng,certifying,and compiling rhe copies.
The responsible authoriry shall comply immediately,if possibie,widi any request tnade pursuant to Uus subdivision,or within five days
of the date of the request,excluding Saturdays,Sundays and legal holidays,if immediate compliance is not possible. If he cannot comply with
the request within that time,he shall so inform t6e individual,and may have an additional five days within which to comply with the request,
excluding Saturdays,Sundays and legal holidays.
Subd.4. Procedure when data is not accurate or complete. An individual may contest the accuracy or completeness of public or
private data conceraing himself. To ezercise this right,an individual shall nodfy in writing the responsible authoriry describing the nature of the
disagreec�nt. The responsible authoriry shall within 30 days either. (a)correct the data found bo be inaccarate or incoroplete and attempt w notify
past recipients of inaccurate or it�complete data,including recipients named by the individual;or(b)notify the individual that he believes the data
to be correct. Data in dispute shall be disclosed only if the i�ividual's statement of disagreement is included with�e disclosed data.
The determination of the responsible authority may be appealed pursuant to the provisions of the administrative procedure act relating
to contested cases.

Re: Structural Inspection of Roof Framing Modifications
Starks Residence, 815 Partenwood Road, Orono, MN
Memo: As requested, I have completed an inspection of the rough framing for the remodeling
work in progress at the existing single family home located at 815 Partenwood Road in
Orono. The purpose for the inspection was to review the roof framing modifications,
es�ecialiy in ihe master be�iruom�uile; family rr�ortY and living roorn alor�g the lakeside
of the house. The existing roof framing in these areas was significantly modified in
" order to create raised flat vaulted ceilings.
The roof framing modifications generally involved cutting the existing 2x6 ceiling joist
members and installing new multiplel-3/4"x 11-7/8"LVL beams along the center
corridor bearing wall of the house and around the perimeter soffit areas. To replace the
restraining force for the hand-framed roof rafter framing originally provided.by the 2x6
ceiling joists, the existing 2x6 roof rafters were braced with new diagona12x4 kickers
� to the new LVL support beams along the corridor bearing wall.
Based on my calculations, the roof framing modifications as described in the above
areas are adequate for all dead loads, snow loads and ti.ind loads as required by the
building code.A[I of the framing inspected was satisfactorily completed
Additional structural work to be completed as noted below:
- install a minimum of 3-2x4 or 2x6.stud posts in the existing ba.rement bearing wall
below all new L�L bearing post locations on the main level. The new bearing studs
and all new blocking in the floor joist space must align on both levels in order to
properly transfer the conce.ntrated loads to the foundation and footing.
- the new WIO steel beam installed in the basement ceiling to support the main floor
�ran�ing requires additional bearing at the east end. This can be accomplished by
acfding a 1-3/�"x 9-1/4"LI�L stud to the ace of�the built-up stud wall that
presently bears the steel beam. The additional stud should be fully glued and
nailed to the face of the studs and shou!d bear on a treated plate in contact with
the concrete slab. The west end of the steel beam bears on treated wood blocking
in a masonry wall pocket. The masonry at the steel beam bearing should be core-
filled with 3000 psi concrete.
