Sec.13.04 RIGHTS OF SIJBJECTS OF DATA
Subd. 1. Type of data. The rights of individual on whom the data is stored or to be stored shall be as set focth in this secdon.
Subd.2. Information required to be given individual. An individual asked to supply private or confidential data concerning himself shall
be informed of: (a)the purpose and intended use of the roquested data within the co(lecting§tate agency,polidcal subdivision,or statewide system;
(b)whether he may refuse or is legally required to supply the requested data;(c)any known consequence arising from his supplying or refusing to supply
private or co�denrial data;and(d)the idendry of other persons or endties authorized by state or federal law to receive the data. This requirement shall
not apply when an individual is asked to supply investigative data,pursuant to section 13.82,subdivision 5,to a law enforcement officer.
The commissioner of revenue mav alace the norice required under this subdivision in the individual income tax or orooertv taz refund
instrucdons instead of on those forms.
Subd. 3. Access to data by individual. Upon request to a responsible authoriry,an individual shall be informed whether he is the subject
of stored data on individuals,and whether it is classified as public,private or confidential. Upon his further request,an individual who is the subject
of stored private or public data on individuals shall be shown the data without any charge to him and, if he desires, shall be infortned of the content
and meaning of that data. After an individual has been shown the private data and informed of its meaning,the data need not be disclosed to him for
siz months thereafter unless a dispute or action pursuant[o this section is pending or additional data on the individual has been collected or created.
The responsible authoriry shall provide copies of the private or public data upon request by the individual subject of the data. The responsible authoriry
may require the requesting person to pay the actual costs of making, cemfying, and compiling the copies.
The responsible authoriry shall comply immediately,if possible,with any request made pursuant to this subdivision,or within five days of
the date of the request,ezcluding Saturdays,Sundays and legal holidays,if immediate compliance is not possible. If he cannot comply with the request
within that time,he shall so inform the individual,and may have an additional five days within which to comply with the request,excluding Saturdays,
Sundays and legal holidays.
Subd.4. Procedure when data is not accurate or complete. An individual may contest the accuracy or completeness of pubtic or private
data conceming himself. To ezercise this right,an individual shall notify in writing the responsible authoriry describing the nature of the disagreement.
The respoasible authoriry shall within 30 days either: (a)corcecc the data found to be inaccurate or incomplete and attempt to notify past recipients of
inaccurate or incomplete data, including recipients named by the individual;or(b)notify the individual that he believes the data to be correct. Data
in dispute shall be disclosed only if the individual's statement of disagreement is included with the disclosed data.
The determination of the responsible authoriry may be appealed pursuant to the provisions of the administretive procedure act relating to
contested cases.

The purpose of this memorandum is to report on existing site conditions and recommended
foundation construction relati��e to the proposed second floor addition.
BACKGROUND
The Palms wish to add a second story to the existing residence at this address.
The City of Orono requires a structural engineering review of the foundation system to ensure
adequate foundation systems for the addition.
OBSERVATIONS
1. Two site visits were conducted by John H. Dahlmeier of J. H. Dahlmeier Engineering
Inc., one with the Palms and the second with Pam Palm and Bruce, the Building Inspector
for the City of Orono.
2. The second floor addition w�ill extend over the entire house except the 10 foot one-story
portion in the front toward the lake, It is my understanding that the intent is to clear span
the second floor and new roof framing from exterior wall to exterior wall. Thus, the new
}oads witl be on exterior walls.

3. Varying foundation conditions were observed around the perimeter of the house.
4. Three test pits were dug by the owner along the left side of the house.
5. Soils at bearing are generally dark clay with a sma11 amount of granular.
6. A recap of the conditions is tisted with a letter designation shown on the propased
condition plan incfuded as Attachment A:
a. Eight inch thick and 7'-4" high concrete block masonry foundation wall with
backfill full height on the wall. Footing is almost 10" wide. The foundation wall
is in good condition with little or no bowing or visible distress, Some distress
noted at the right front corner.
b. Eight inch thick concrete masonry foundation wall 24 - 30" deep wiih 10" wide
footing backfilled above the foundation wall.
c. Similar to b.

e. Eight inch concrete foundation 7'-4" high founc3ation conerete masonry wa11 ebout
24" - 30" deep with crawtspace adjacent backfilled down about 24" on interior
and over foundation wall at exterior,
f. Eight inch concrete masonry foundation wall approximately 18" below grade with
10" wide footing and 7'-4" above grade with crawlspace Iike "e".
g. 7'-4" high, 8" concrete masonry foundation wall exposed to exterior as walkout
type basement wall with 18" of foundation below grade on 10" wide footing.
h. Eight inch masonry foundation wall 7'-4" high exposed with approximately ]6"
wall below grade with ]0" wide footing.
i. Eight inch concrete cnasonry foundation wall above fluor slab about 3 feet with
backfill on exterior providing frast protection to the faundation system
j. Eight inch cancrete foundation 7'-4" high foundatian concrete rnasonry wal! with
crawlspace adjacent backfilled down about 24",
7. Significant �ssues with resp�ct to an adequate fa�:ndatian for the second fl4or addition
are.
a. Inadequate frost protection in many locations.
b. Inadequate footing width for second floor laads and load bearin� walls.

8. Generally, foundation walls appear to be in good condition with little bowing.
9. Corrective measures will be required in order to support a second floor addition as
follows (the sub letters match the above-described conditions}:
a. Excavate to footing. Underpin footing to provide 2 foot wide footing continuous.
Reinforce 8" masonry foundation wall with #S vertical @ 32" fil{ed with grout at
reinforcing.
b,c,d. Excavate and provide underpinning to frost depth of 42" 2 feet wide if load
bearing. If not load bearing, no corrective measures are required.
e,i,j. No repairs are required.
f,g,h Underpin with 2 foot wide footing to frost depth of 42".
10. General corrective measures include:
a. Regrading to pravide positive drainage away from foundation walls the entire
perimeter of house.
b. Ensure that grade is at least 8" below top of foundation.
11. A beam column system will be needed along the street sicle entrance wall to carry load
from the second floor. Conective measures will not be needed along this watl. A
column and column footing will need to be installed near the center of the span for
support (in the vicinity of the cistern}.
12. A cistern was discovered under the street side entrance with severe erosion creating a
large cavity outside as well as the interior of the cistern. This cistern should be filled
with coarse granular material for safety reasans.
13. These corrective measures are general in nature but adeyuate for estimating purposes to
get the pro}ect started.
14. 3.H. Dahlmeier Engineering Inc. is available to provide additional engineering for
construction.

The purpose of this memorandum is to provide observations made during a site visit to the
proj ect.
BACKGROUND
J. H. Dahlmeier Engineering Inc. prepared a Memorandum Report providing information
regarding the structural adequacy of the foundation system for the addition.
OBSERVATIONS AND COMMENTS
1. Based on on-site conditions, corrective measures recommended for condition a have been
modified.
2. It has been recommended that a pilaster system be installed to support the wall rather
than the in-wall reinforcing.
SPECIAL INSPECTION
3. A special inspection was conducted on November 24, 1998 to review conditions on the
site. The following observations were made:
a. The pilaster system and footing have been constructed.
b. Reinforcing is in place.
c. Concrete fill is scheduled.

4. Condition d at the front will be continued to provide full underpinning eliminating the
need for the column, column footing and the clear span above.
5. All the work is progressing in accordance with the recommendations.
