During routine inspections by the Minnehaha Creek Watershed District(MCWD)staff on August
21, 2009,at the above mentioned location the following violations were recorded:

• Send or deliver a completed Combined Joint Notification permit application with an original
• signature for each project
• Site plans showing contour elevations of each site and adjacent sites
• Indicate on each site plan where you propose to put silt fence and a temporary rock/gravel

• Silt fence is required in front of the each site,along the road, and on the bottom of steep slopes

Scott Thomas issuance of a stop work order,criminal misdemeanor prosecution,and fees for additional
inspections and legal costs.Working without a permit where one is required is in violation of
MCWD Rules and is a misdemeanor subject to penalty by law.Work must stop until a permit is

Conditions; Project to be completed as described in plans submitted to the
MCWD office on August 29, 2003 according to the provisions of
this permit.
• Seed and mulch/install erosion control blanket on all disturbed
areas until permanently stabilized with actively growing
vegetation by 09/03/03.
• Properly install and maintain silt fence at the bottom of all
downhill slopes if seeding and mulching/installing erosion
control blanket is delayed past 09/03/03 until the disturbed
areas are restabilized
• Silt fence must be orange In color
• When the site is restablilized and the MCWD staff has
performed a final inspection, all silt fences must be removed
