We have inspected the existing roofing on the home at the above referenced location.
The existing roofing consists of medium shakes installed at a 10"exposure. During our
roof inspection we noticed that the existing cedar is deteriorated and in need of
replacement. Les Jones Roofing will be bidding on the complete tear off and reroof of
the garage addition. We propose to do the following work.
SPECIFICATIONS
1. We will tear off the existing roofing on the garage addition. All debris will be
deaned up and hauled away.
During the tear off process, debris may sift down through the roof boards.
Therefore, we highly recommend that you remove or cover any items that may
be stored in the attic area. We cannot be held liable (referring to the debris
sifting through)for any interior contents during this process.
2. Certainteed's WinterGuard will be installed up the first six feet at the eaves.
3. One layer.of 30#felt will be mechanically fastened over the remaining,exposed
roof area.
4. New galvanized metal 5x7 shingle tins will be installed.
5. New prefinished metal Drip edge will be installed.
6. New copper ridge metal will be installed.

6. We will install new #1 Blue Label 24"medium handsplit shakes, installed at a 10"
exposure to the weather. All cedar roofing will be installed per the Red Cedar
•" Shingle and Handsplit Bureau specifications.
7. One layer of 18"felt will be mechanically fastened between each course of
cedar.
8. New cedar ridge will be installed.
9. Les Jones Roofing will issue a 10 year watertight warranty.

We will pressure wash the the roof. This work will be done on a time and materials
basis. Our rate is $75.00 per hour, per man, plus materials. We estimate our cleaning
rate at one square per hour.

Our rate is $75.00 per hour, per man, plus materials. We estimate replacing two tiles
per hour. We estimate replacement of twenty-five to thirty-five tiles will be needed.

Dumpster - While your job is in progress, we may need to place a dumpster in your
driveway. If the dumpster cannot be placed in your driveway, there may be an
additional charge to place it in the street, if this is allowed in your area. This dumpster
is for Les Jones Roofing's construction debris only. The landfill will not accept any type
of household debris.

Trucks - While your job is in progress, we will need to utilize your driveway for
delivery of equipment and materials. Our crew will plan to park in your driveway for
access to their tools and equipment. If this is a problem, please notify us to request a
different parking arrangement.
Les Jones Roofing is licensed, bonded and insured.
Necessary building permits will be obtained by Les Jones Roofing.

PAYMENT TERMS
We will require a 30% down payment for this work. Please enclose your 30% down
payment along with your signed copy of this contract. If the 30% is not endosed, we
will notify you of your anticipated start date. Any project with an outstanding down
payment will be subject to cancellation and will be rescheduled when the down payment
is received.
We will send monthly progress invoices at the end of each month. Payment of the
amount reflected In the monthly progress invoice is due upon receipt.

Sec.I3.04 RIGHTS OF SUBJECTS OF DATA
Subd. 1. Type of data. The rights of individual on whom the data is stored or to be stored shall be as sec forth In this section.
Subd,3. Information requdred to be given iadlvidtd. An individual asked to supply private or conftdendal dam concerning himself shell
be informed of: (a)the purpose and intended use of the requested data within the collecting hate agency,political subdivision,or statewide system;
(b)whether he may refuse or is legally required to supply the requested data;(c)any!mown consequence arising from his supplying or refusing to supply
private or confidential data;and(d)the identity of otherpersocs or entities authorized by state or federal law to receive the data.,This requirement shall
not apply when an irdividual is asked to supply investigadve data,putsuree to section 13.82.subdivision S,to a law enforcement officer.
The commissioner of revenue may Glace the notice required ceder this subdivision in the individual income tax or property tax refund
instructions(named of on those forms.

Subd.3. Access to data by individual. Upon request to a responsible authority,an individual shall be informed whether he is the subject
of stored data on individuals,and whether it is classified as public,private or confidential. Upon his further request an individual who is the subject
of stored private or public data on individuals shall be shown the data without any charge to him and;if he desires,shall be informed of the content
and meaning of that data. After an individual has been shown the private data and informed of its meaning,the data need not be disclosed to him for
six months thereafter unless a dispute or action putsusnt to this section is pending or additional data on the individual has been collected or created.
The responsible authority shalt provide copies of the private or public data upon request by the individual subject of the data. The responsible authority
may require the requesting person to pay the actual costs of making,ce eying.and compiling the copies.
The responsible authority shall comply inunediaicly.if possible.with any request made pursuant to this subdivision,or within five days of
the date of the request.excluding Saturdays,Sundays and legal holidays,if immediate compliance is not possible. If he cannot comply with the request
within that time.he shall so inform the individual,and may have an addidc:3i five days within which to comply with the request.excluding Saturdays,
Sundays and legal holidays.
Subd.a. Procedure when data is not accurate or complete. An individual tray contest the accuracy or completeness of public or private
dam concerning himself. To exercise this right.an individual shall notify is writing the responsible authority deseribiing the nature of the disagreement.
The responsible authority shall within 30 days either; (a)correct the dam found to be inaccurate or incomplete and attempt to notify past recipients of
inaccurate or incomplete dam,including recipients named by the individual;or(b)notify the individual that he believes the data to be correct. Data
in dispute shall be disclosed only if the individual's statement of disagreement is included with the disclosed data.
The determinadon of the responsible authority may be appealed pursuant to the provisions of the administrative procedure act relating to
contested cases. .
