Melanie,
Here are the items to be addressed and that are addressed.
1. Mulch was placed over disturbed ROW soil.
2.Waiting to hear back from Terrance on buffer vegetation.
3. Boulders were flipped to 4' or less in height.
4. Drainage structure to be addressed by John and Jeff
5. Street being scraped and swept right now.
6.The 4 spruce west of driveway were not touched, so no replacement necessary. 3 4-5 stem clump birch trees were
planted to the East of driveway. Most cities count each trunk as a tree. 3 evergreens were also planted east of
driveway. One more was planted to the west as I feel that it would be pretty full to the west with the birch clumps.
Please, let me know if there is anything else you need us to address.

Our engineer reviewed the as-built survey and conducted a site inspection today at 190 Orono Orchard
Road. Please see his detailed comments below. The identified issues must be addressed prior to issuance
of a final Certificate of Occupancy for the new home. Further, all exposed soil should be vegetated and
stabilized (established) prior to the final CO.
It appears the items needing resolution described in detail below are as follows:
1. Vegetation of the disturbed right-of-way area
2. Buffer vegetation
3. Engineered design for retaining walls
4. Drainage structure maintenance
5. Street cleaning
6. Tree planting
Regarding the drainage structure, please provide clarification if a utility/drainage easement exists over
this area. This easement would be included in the title work and, if it exists, should be reflected on the
survey.
I have also attached the builder acknowledgement form initialed at the time of the permit issuance. I
highlighted the notes regarding the drainage inlet and retaining walls.

I've reviewed the as built and conducted a site visit. I offer the following comments:
1. The disturbed area of the ROW should be vegetated. (see Photo)

2. Vegetation in the Buffer has not germinated yet, if planted.

3. The retaining wall/walls in next to the patio exceed 4ft in height. I could not find a copy of the
engineered design for the walls signed by a licensed engineer as required by the City.

4. The drainage structure on the property is overgrown and failing. It needs to be cleared. (Note- I had
offered a several occasions to do this during construction. Just needed the builder to call me and let me
know a good time. It will now be very difficult for the city to access the site without tearing up the new
sod). If there is an easement over this structure it should be shown on the survey.

5. Contractor should complete a final cleaning of the street to remove dirt and construction debris.

6. Not all of the trees on the approved plan appear to have been planted. To the east of the driveway
the plan called for 4 coniferous and 3 deciduous. There are 3 coniferous and 4 deciduous. To the west
of the driveway the plan called for 4 coniferous. Only one coniferous was planted.
