RE: 645 Old Long Lake Road, SAC Credit
In the process of issuing a New Home permit here at the City, we discovered that
645 Old Long Lake Road had a SAC Credit logged on the books from 2/22/01,
under Permit #3548.
In doing further research it was determined this was reported in error back in
2001. This property never paid SAC nor had SAC Grandfathered in.
Please remove this "Credit" from your books. Also, note: we will be
collecting the SAC Charge with the New Home permit being issued this year
2005, and reporting this when that permit is picked up.

This form is for reporting all SAC-related demolitions to be placed on record as potential SAC credits.
.... Attach Form MCES 92C for all Commercial facilities in order to show how number of SAC units of
credit was determined.
.... Reduce the SAC units of credit for Apartments by 20%and Public Housing by 25%in order to adjust
for previously reduced charges.
.... All Industrial facilities must have a MCES SAC determination letter attached. Use Form MCES 92F
to apply for the determination letter.
