General Instructions:
1. You may be required to obtain other permits, i.e.: well abandonment, sewer, etc.
2. Work must not begin unless the permit card is available on the job site.
3. A 24-48 hour notice is required for all inspections. Call (952)249-4600.
4. Sewer must be discontinued at the City service by qualified contractor before demo permit is
issued.

In return for issuance of said Demolition Permit, the undersigned owner hereby agrees as follows:
1. The structure(s) shall be kept enclosed and/or secured until such time as demolition is
complete.

2. Demolition debris will be kept off adjoining property and/or the public rights-of way unless
specific prior approval is obtained in writing for temporary use thereof.
3. Foundations shall be completely removed from the ground.
4. All demolition debris shall be completely disposed of off site in accordance with all
applicable PCA requirements.
5. Water wells must be abandoned in accordance with State Health Department regulations.
6. Inspection required when all debris has been removed,before backfilling.
7. Within 5 working days of superstructure removal, a final inspection shall be requested. The
site shall be left clean and clear of all debris,with any excavation filled with earth level with
the adjacent ground elevation(except when such excavation is to be used as part of a new
building and such new building is actually under construction).
8. Septic systems must be abandoned per Minnesota Rules Chapter 7080. All septic tanks
must be pumped, crushed and filled with native soils. An inspection is required after the
tanks are pumped and before the tanks are crushed and filled.
9. The undersigned owner shall and hereby does indemnify and hold harmless the City of
Orono, its agents, employees and assigns from and against all claims, damages, losses or
expenses, including attorney fees, against the City, its agents, employees and assigns arising
out of or resulting from the demolition described herein as performed by the property owner,
his employees, agents, subcontractors or assigns.

Principal Dwelling Demolition Permit
ZONING DISCLOSURE & DECLARATION
To the property owner: Demolition of the principal dwelling structure on a property may automatically terminate
certain rights which may have accrued to the property by virtue of the continued existence of that building.
• Rebuilding on a substandard lot of record (i.e. a lot that does not meet the zoning district required lot area or
width standards)will, with few exceptions, require variance approval by the City Council, and such approval is
neither automatic nor guaranteed but requires that a hardship be demonstrated.
• Additionally, all current zoning standards will have to be met by the new principal dwelling including setbacks,
lot coverage by structures, hardcover(impervious surface), height limits, etc.
• Where municipal sewer is not available, provision of two (2) sites for a conforming on-site sewage treatment
system is mandatory.
• Unless specifically approved by the City, all accessory structures must be removed at the time of principal
dwelling demolition. This also applies to seasonal and permanent docks,which may not be re-installed until a
new principal dwelling has reached the framing stage.
The following information is presented for the purposes of advising the property owner of the implications of
removal of the principal dwelling on the property:

It was a pleasure to speak with you today. In order to continue our review of demolition permit application
2011-00947, we need the following submitted:
• A survey, aerial photo or sketch showing all structures on the property. Note which structures are to be
demolished.
• A survey, aerial photo or sketch showing proposed erosion control measures in accordance with
Chapter 79, Construction Site Runoff Control.
After we receive the above items, we will continue our review of the demolition permit application.
We will call to notify you the permit is ready for pickup. At that time we will need the attached two forms
(escrow agreement and zoning disclosure) signed and brought in, along with the $2,000 escrow.

DEMOLITION PERMIT 2011-00947
ESCROW AGREEMENT
AGREEMENT made this 3 day of /,Jv , 20 t'/ , by and between the CITY OF ORONO, a
Minnesota municipal corporation ("City") and Jam &Heidi Williams ("Owners").
Recitals
1. An application for a demolition permit has been filed for the principal structure and one (1) accessory
structure located at 840 Old Crystal Bay Road S ("Subject Property") legally described as described on Exhibit "A"
attached hereto.
2. In conjunction with the demolition erosion control measures are required as shown on attached
Exhibit"B".
NOW,THEREOFRE,THE PARTIES AGREE AS FOLLOWS:
1. DEPOSIT OF ESCROW FUNDS. Contemporaneously with the execution of this Escrow Agreement,
the Owners shall deposit $2,000 with the City. All accrued interest, if any, shall be paid to the City to reimburse the
City for its cost in administering the escrow account.
2. PURPOSE OF ESCROW. The purpose of the escrow is to guarantee reimbursement to the City for
all out-of-pocket costs (including planning, engineering, or legal consultant review)the City has incurred to assure that
the erosion control measures are installed and maintained so the property complies with the provisions of Orono City
Code Chapter 79. The financial security may also be used by the City to eliminate any hazardous conditions
associated with the work and to repair any damage to public property or infrastructure that is caused by the work. If
compliance with the approved Land Disturbance Permit is not accomplished within the allowable time period, the City
may bring the project into compliance by use of the security.
3. RIGHT OF ENTRY. The Owners hereby grant the City, its agents, employees, officers and
contractors the right to enter the property to perform all work and inspections deemed appropriate by the City in
conjunction with the required erosion control measures, including but not limited to constructing or completing any and
all of the agreed upon improvements should the Owners' contractor not complete those improvements by the date
specified herein.
3. MONTHLY BILLING. As the City receives consultant bills for incurred costs, the City will in turn
send a bill to the Owners. Owners shall be responsible for payment to the City within 30 days of the Owners' receipt
of bill.
4. DISBURSEMENT FROM ESCROW ACCOUNT. In the event that the Owners do not make payment
to the City within the timeframe outlined in#3 above, shall issue a Stop Work Order until the Owners pay all expenses
invoiced pursuant to #3. The City may draw from the escrow account without further approval of the Owners to
reimburse the City for eligible expenses the City has incurred.
5. CLOSING ESCROW. Any remaining amount of the financial security deposited with the City for
faithful performance of the plans and specifications and any related remedial work will be released after the
completion and inspection of all such measures and the establishment of final stabilization for the Subject Property.
6. CERTIFY UNPAID CHARGES. If the project is abandoned by Owners, or if the eligible expenses
incurred y the City exceed the amount in escrow, the City shall have the right to certify the unpaid balance to the
subj property pursuant to Minn. Stat. §§415.01 and 366.012.

Recitals
1. A building permit application has been filed for a building permit located at 840 Old Crystal Bay
Road South, the ("Subject Property"), legally described as the South 226.8 feet of the North 391.8 feet of the
West 60 Rods of the Northeast Quarter of Section 9, Township 117, Range 23, Hennepin County Minnesota.
2. Owners request the City to review this application.
3. The City will commence its review of the application and incur costs associated with said review
only if the Owner establishes an escrow to ensure reimbursement to the City of its costs.
4. The scope of the project which is the subject of building permit #2011-01003 triggers the
application of the City's wetland buffer requirement as established by City Zoning Code Article XI Wetlands
Protection will commence its review of the application and incur costs associated with said review only if the
Owner establishes an escrow to ensure reimbursement to the City of its costs.
NOW THEREFORE,THE PARTIES AGREE AS FOLLOWS:
1. DEPOSIT OF ESCROW FUNDS. Contemporaneously with the execution of this Escrow
Agreement, the Owners shall deposit$5,500 with the City. All accrued interest, if any, shall be paid to the City to
reimburse the City for its cost in administering the escrow account.
2. PURPOSE OF ESCROW. The purpose of the escrow is to guarantee the establishment and
development of the required native wetland buffer and reimbursement to the City for all out-of-pocket costs the
City has incurred (including planning, engineering, in excess of $500, or legal consultant review) or will incur in
reviewing the plan. Eligible expenses shall be consistent with expenses the Owners would be responsible for
under a building permit application and as part of the establishment of the wetland buffer. The escrow will also
guarantee reimbursement to the City for all out-of-pocket costs the City has incurred to assure that the work is
completed in accordance with the Stormwater Pollution Prevention Plan and the provisions of Orono City Code
Chapter 79. The financial security may also be used by the City to eliminate any hazardous conditions associated
with the work and to repair any damage to public property or infrastructure that is caused by the work (including
planning, engineering, or legal consultant review) associated with building permit #2011-01003 if compliance
with the approved building permit and wetland buffer establishment is not accomplished.
3. MONTHLY BILLING. As the City receives consultant bills for incurred costs, the City will in turn
send a bill to the Owners. Owners shall be responsible for payment to the City within 30 days of the Owners'
receipt of bill.
4. DISBURSEMENT FROM ESCROW ACCOUNT. In the event that the Owners do not make payment
to the City within the timeframe outlined in#3 above, shall issue a Stop Work Order untiltheOwners pay all expenses
invoiced pursuant to #3. The City may draw from the escrow account without further approval of the Owners to
reimburse the City for eligible expenses the City has incurred.
5. CLOSING ESCROW. The Balance on deposit in the escrow, if any, shall be returned to the
Owners when the review has been completed and written notification is received from the Owners requesting the
funds.
6. CERTIFY UNPAID CHARGES. If the project is abandoned by Owners, or if the eligible expenses
incurred by the City exceed the amount in escrow, the City shall have the right to certify the unpaid bale•ce t. the

ESCROW AGREEMENT

AGREEMENT made this 3 I day of • , 20`i , by and between the CITY OF ORONO, a
Minnesota municipal corporation("City")and Jam &Heidi Williams("Owners").
Recitals
1. An application for a demolition permit has been filed for the principal structure and one(1) accessory
structure located at 840 Old Crystal Bay Road S ("Subject Property") legally described as described on Exhibit "A"
attached hereto.
2. In conjunction with the demolition erosion control measures are required as shown on attached
Exhibit"B".
NOW,THEREOFRE,THE PARTIES AGREE AS FOLLOWS:
1. DEPOSIT OF ESCROW FUNDS. Contemporaneously with the execution of this Escrow Agreement,
the Owners shall deposit$2,000 with the City. All accrued interest, if any, shall be paid to the City to reimburse the
City for its cost in administering the escrow account.
2. PURPOSE OF ESCROW. The purpose of the escrow is to guarantee reimbursement to the City for
all out-of-pocket costs(including planning, engineering, or legal consultant review)the City has incurred to assure that
the erosion control measures are installed and maintained so the property complies with the provisions of Orono City
Code Chapter 79. The financial security may also be used by the City to eliminate any hazardous conditions
associated with the work and to repair any damage to public property or infrastructure that is caused by the work. If
compliance with the approved Land Disturbance Permit is not accomplished within the allowable time period,the City
may bring the project into compliance by use of the security.
3. RIGHT OF ENTRY. The Owners hereby grant the City, its agents, employees, officers and
contractors the right to enter the property to perform all work and inspections deemed appropriate by the City in
conjunction with the required erosion control measures, including but not limited to constructing or completing any and
all of the agreed upon improvements should the Owners' contractor not complete those improvements by the date
specified herein.
3. MONTHLY BILLING. As the City receives consultant bills for incurred costs, the City will in turn
send a bill to the Owners. Owners shall be responsible for payment to the City within 30 days of the Owners' receipt
of bill.
4. DISBURSEMENT FROM ESCROW ACCOUNT. In the event that the Owners do not make payment
to the City within the timeframe outlined in#3 above, shall issue a Stop Work Order until the Owners pay all expenses
invoiced pursuant to #3. The City may draw from the escrow account without further approval of the Owners to

5. CLOSING ESCROW. Any remaining amount of the financial security deposited with the City for
faithful performance of the plans and specifications and any related remedial work will be released after the
completion and inspection of all such measures and the establishment of final stabilization for the Subject Property.
6. CERTIFY UNPAID CHARGES. If the project is abandoned by Owners, or if the eligible expenses
incurred City exceed the amount in escrow, the City shall have the right to certify the unpaid balance to the
subj property ursuant to Minn.Stat.§§415.01 and 366.012.
