, . You may apply for mechanical permits by mail or in person at the Cit!
offices. Mailed-in permits are subject to the postage and handling feeE
shown below.
2 . Permit cards will be sent by return mail the same day the application ic-
received. PERMITS ARE NOT VALID UNTIL YOU RECEIVE A PERMIT. WORK MUST NO"_
BEGIN UNTIL THE PERMIT CARD IS POSTED ON THE JOB SITE.
3 . When any new construction or remodeling is involved, a separate buildinc
permit must be obtained.
4 . All work must be done in accordance with State Building Code requirements.
5. All work must be inspected (rough-in and final ). Call 473-7357. 24-hour
notice required.
6. House Heating Test Record must be submitted before final.
INSTRUCTIONS Complete all items on this application. Compute the permit fee.
Sign and date the certification. INCOMPLETE APPLICATIONS WILL NOT BE PROCESSED.

The plans for the above installation have been reviewed pursuant to Minnesota Statutes, 1984, Chapter
299F.19.
Preliminary approval is given for the aforementioned project subject to compliance with the provisions
of Minnesota Statutes, State Fire Marshall Regidations, and local ordinances and permits. Construction
shall be in conformance with Article 79, Flammable and Combustible Liquids, contained in Minnesota
Uniform Fire Code (1988 edition as amended).
Final approval will be given follovvirrg an itI,pertion of the facility by either your area local fire authority
or Deputy State Fire Marshall.
Approval of the project desrribed in this letter does not relieve the applicant of responsibility to other
Federal, State or local agencies regarding adhere,1,rp to regulations or the need to obtain necessary approval.
