Zoning Application 08-3349 for the Orono School District, 685 and/or 795 Old Crystal Bay Road resurfaced track within HS Stadium, replaced stadium field grass w/ artificial turf and created a level area south of the stadium w/ excess material. The permit was pulled in May of 2008 with a note from Evelyn Turner that read: Wetland buffer improvement plan and easement required before field is put in use. I'm wondering what your records show? Is the attached declaration from the MCWD fulfill Evelyn's requirement or do we need to prepare something else for them to sign? I look forward to hearing from you. WHEREAS application has been made for permit from the MCWD PP pursuant to MCWD.Rule'N;Stormwater Management;and WHEREAS,the property that is the subject of the permit is legaliy' described in the Declaration into which this Attachment is incorporated,and the facilities on the property to which these maintenance requirem ants apply are as follows: 1. Bio-filtration area#2: south of School stadium and east of gravel drive WHEREAS,MCWD Rule N provides that a maintenanc a agreement shall be submitted for stormwater treatment ponds,outlet structure for such ponds, culverts,outfall structures and all'other stormwater facilities, d that the maintenance agreement shall specify methods,schedule and responsible parties for maintenance and must include of a minimum,the elements contained in the District's Maintenance Agreement Form. NOW,THEREFORE: 1. The property owner shall inspect the stormwater retention and treatment basins)!at a minimum of once a year to determine if the basin's retention and treatment-characteristics are adequate. A storage treatment basin will be considered inadequate if sediment has decreased the wit storage volume by 1/z of its original design volume. Based on this inspection, ifhe stormwater basin(s)is identified for sediment cleanout,the property owner shall restore the basin(s)to its original design contours within one year of the inspection date. j 2. The property owner shall inspect the grit chambers,sump catch basins,sump manholes, outlet structures,culverts, outfall stru es or other stormwater facilities for the project in the spring and fall of eacl L year.
