Non permitted events will not be allowed. 5 Sec.13.04 RIGHTS OF SUBJECTS OF DATA Subd. 1. Type of data. The rights of individual on whom the data is stored or to be stored shall be as set forth in this section. Subd.2. Information required to be given individual. An individual asked to supply private or confidential data concerning himself shall be informed of. (a)the purpose and intended use of the requested data within the collecting slate agency,political subdivision,or statewide system;(b)whether he may refuse or is legally required to supply the requested data;(c)any known consequence arising from his supplying or refusing to supply private or confidential data;and(d)the identity of other persons or entities authorized by state or federal law to receive the data. This requirement shall not apply when an individual is asked to supply investigative data,pursuant to section 13.82,subdivision 5,to a law enforcement officer. The commissioner of revenue may place the notice required under this subdivision in the individual income tax or properly tax refund instructions instead of on those forms. Subd.3. Access to data by individual. Upon request to a responsible authority,an individual shall be informed whether he is the subject of stored data on individuals,and whether it is classified as public,private or confidential. Upon his further request,an individual who is the subject of stored private or public data on individuals shall be shown the data without any charge to him and,if he desires,shall be informed of the content and meaning of that data. After an individual has been shown the private data and informed of its meaning,the data need not be disclosed to him for six months thereafter unless a dispute or action pursuant to this section is pending or additional data on the individual has been collected or created. The responsible authority shall provide copies of the private or public data upon request by the individual subject of the data. The responsible authority may require the requesting person to pay the actual costs of making,certifying,and compiling the copies. The responsible authority shall comply immediately,if possible,with any request made pursuant to this subdivision,or within five days of the date of the request,excluding Saturdays,Sundays and legal holidays,if immediate compliance is not possible. If he cannot comply with the request within that time,he shall so inform the individual,and may have an additional five days within which to comply with the request, excluding Saturdays,Sundays and legal holidays. Subd.4. Procedure when data is not accurate or complete. An individual may contest the accuracy or completeness of public or private data concerning himself. To exercise this right,an individual shall notify in writing the responsible authority describing the nature of the disagreement. The responsible authority shall within 30 days either. (a)correct the data found to be inaccurate or incomplete and attempt to notify past recipients of inaccurate or incomplete data,including recipients named by the individual;or(b)notify the individual that he believes the data to be correct. Data in dispute shall be disclosed only if the individual's statement of disagreement is included with the disclosed data. The determination of the responsible authority may be appealed pursuant to the provisions of the administrative procedure act relating to contested cases. Dahlmeier Engineering Inc. August 22, 1997 Walter Holzer 1130 North Shore Drive Orono,MN 55364 Re: Garage Structure 1130 North Shore Drive Orono,MN Commission No. 97167 Dear Mr. Holzer: ASSIGNMENT J. H. Dahlmeier Engineering Inc. has been retained to provide a structural engineering review of the foundation system under construction for the garage structure to be constructed at 1130 North Shore Drive, Orono, MN as directed by Walter Holzer, owner of the property. BACKGROUND Walter Holzer intends to construct a garage structure Group M occupancy. The City of Orono Building Inspections Department requires a structural engineering review of the foundation construction. OBSERVATIONS AND COMMENTS 1. Drawings for the structure include six sheets dated 2/24/97, Plan No. C103056, prepared by Minnetonka Design, Inc. A copy of the roof truss design has been furnished to me. The foundation layout using a thickened slab edge has been Frovided to me. The structure will be 30' x 33' with a storage space in the attic space within the roof truss system. Thickened slab method is being used instead of the full depth footings and foundation wall shown on the drawings. 2. The thickened slab concrete foundation system consists of a 4" thick slab with a thickened edge 12" deep and 12" wide and tapered at a 45 degree angle up to the slab thickness on all four perimeter sides with two#4 running the long direction and three#4 running the short direction in the slab. 3. The thickened slab has two #4 bars stacked the entire perimeter of the building. The slab is constructed on 20"of pit run clean gravel. 5. The excavation area is oversized 6 feet on all sides on the clay subgrade. The clay surface is pitched to the edges. 6. The concrete delivery ticket indicates the concrete was furnished by Cemstone Products Company with a specified strength of 4000 psi at 28 days with 6%air entrainment. ENGINEERING ANALYSIS 7. The foundation under construction meets the requirements for a group M occupancy structure using thickened slab construction. 8. The system is structurally adequate to support the design loads as required by the Code. 9. A thickened slab foundation system does not go down to frost, therefore movements as much as 3"to 4"can be expected at times during winter months for this type of construction. 10. The thick gravel bed will help minimize the amount of movement. Drainage of this gravel fill is very important. It is strongly recommended that a draintile system be installed daylighting away from the structure. This will ensure that the gravel fill is kept as dry as possible. 11. A minimum amount of steel is being installed in the thickened slab portion and in the slab. It is recommended that the slab be saw cut into relatively square units, in this case six units would be appropriate. Anticipated loads are light. GENERAL 12.
