Non permitted events will not be allowed. 5 Sec.13.04 RIGHTS OF SUBJECTS OF DATA Subd. 1. Type of data. The rights of individual on whom the data is stored or to be stored shall be as set forth in this section. Subd.2. Information required to be given individual. An individual asked to supply private or confidential data concerning himself shall be informed of: (a)the purpose and intended use of the requested data within the collecting state agency,political subdivision,or statewide system;(b)whether he may refuse or is legally required to supply the requested data;(c)any known consequence arising from his supplying or refusing to supply private or confidential dam;and(d)the identity of other persons or entities authorized by state or federal law to receive the data. This requirement shall not apply when an individual is asked to supply investigative data, pursuant to section 13.82, subdivision 5, to a law enforcement officer. The commissioner of revenue may place the notice reouired under this subdivision in the individual income tax or orooerty tax refund instructions instead of on those forms. Subd. 3. Access to data by individual. Upon request to a responsible authority,an individual shall be informed whether he is the subject of stored data on individuals,and whether it is classified as public,private or confidential. Upon his further request,an individual who is the subject of stored private or public data on individuals shall be shown the data without any charge to him and, if he desires,shall be informed of the content and meaning of that data. After an individual has been shown the private data and informed of its meaning,the data need not be disclosed to him for six months thereafter unless a dispute or action pursuant to this section is pending or additional data on the individual has been collected or created. The responsible authority shall provide copies of the private or public dam upon request by the individual subject of the data. The responsible authority may require the requesting person to pay the actual costs of making,certifying,and compiling the copies. The responsible authority shall comply immediately,if possible,with any request made pursuant to this subdivision,or within five days of the date of the request,excluding Saturdays,Sundays and legal holidays,if immediate compliance is not possible. If he cannot comply with the request within that time, he shall so inform the individual,and may have an additional five days within which to comply with the request, excluding Saturdays,Sundays and legal holidays. Subd.4. Procedure when data is not accurate or complete. An individual may contest the accuracy or completeness of public or private dam concerning himself. To exercise this right,an individual shall notify in writing the responsible authority describing the nature of the disagreement. The responsible authority shall within 30 days either: (a)correct the dam found to be inaccurate or incomplete and attempt to notify past recipients of inaccurate or incomplete data,including recipients named by the individual;or(b)notify the individual that he believes the data to be correct. Data in dispute shall be disclosed only if the individual's statement of disagreement is included with the disclosed data. The determination of the responsible authority may be appealed pursuant to the provisions of the administrative procedure act relating to contested cases. Demolition; B. Remove the wood stoop. C. The cost of a dumpster is included D. The existing soffiting for the south side of the master bedroom, the west side of the "hat" room and dining room, and the north side of the living room will be removed back to the wall 3. Foundation; A. Three - 20" wide and two 12" wide pier footings will be dug and poured as per the drawings, and a galvanized footing post hanger will be installed in each. B. A 6'by 6" treated post will support the area over the garage entrance door. The balance of footings will support the deck with treated 6 by 6 posts. 4. Deck; (See Option #1, which was approved) A. 2x6 Treated joists, 16" on center will rest on a 2x6 treated ledger which is bolted into the home. Each joists will rest in a 2x6 joist hanger against the home B. A double 2x12 treated beam will run the length of the deck, the joists will cantilever beyond the beam approximately 1'0' C. Galvanized flashing will be installed at the siding line and overlapp the new decking D. Decking to be 2x6 Select s4s cedar. E. A 2x8" cedar rim fascia will be installed on the outside rim 1 v 5. Steps; A. Stringers to be 2x12 s4s cedar, treads to be 2x6 Select cedar, with 1x8 cedar risers, all resting on the two 12" footings. 6. Roof, A. The existing roof has a"mid-life sag", which is visible from the ground it is our intention to open up the roof at the point where the new and old roofs meet and try to "shim" up the existing roof boards at that point. We may not be able to eliminate all of the sag (which is the same for my sag...),but we hope to greatly improve the look. The existing roof will be left open underneath the new rafters for good ventilation. B. 2x6 Roof rafters will be installed 16" on center to form a shed roof slope. As well, 2x6- 16" on center will form the window well. 2x4's will form an additional knee wall at the existing wall line for additional support for the rafters. C. A double 2x12 SPF header will run the length of the porch. All rafters and ceiling joists will rest on it. D. Ceiling joists to be 2x4", 12" on center D. All will be covered with 1/2" CDX plywood and 15# Asphalt paper. 7. Soffit and Fascia; A. 1x6" Cedar fascia, 1x3" Cedar frieze and 3/8" Cedar plywood with continuous soffit venting. B. Additional 1x3 Frieze will be added to the two gables and to the one dormer above. This will not continue around the sides or back of the building at this time. 8. Roofing; A. Based on a 25 year 240# 3-tab shingles B. New Roofing will be installed on the new addition,the front and back of the existing home and the two front and one rear dormer roofs. C. Tear off the existing roofing and dispose D. Does not include any gutter work at this time E. Cover all with 15#Asphalt paper, and Weather Watch over the eaves and down the valleys F. Install 120 lineal feet of valley flashing, one 4" plumbing stack and one 2" stack G. 140 ft of ridge venting H. Dormer flashings, step flashing at chimneys and porch I. 60 mil EDPM rubber membrane at the front flat roof 9. Porch trim; A. We will wrap the 2x12 header in cedar plywood or boards, suitable for painting B. The ceiling will be 3/8" thick Southern Yellow Pine beaded porch plywood running perpendicular to the house wall, C. Posts to be solid turned , resting off the deck by means of galvanized brackets to protect against rot. D. Handrail to be site-made using cedar 2 E. Spindles to be 1 1/2" by 1 1/2" square cedar. F. The stairway and"widows walk" handrail to have the same handrail and spindles, but the corner and newel posts will be square cedar with a cedar ball-finial. Note: The "widows' walk" is only decorative. It is not the intention of The Carpenter's to offer any suggestion as to the future use of this upper deck, neither do we assume any responsibility should the wife take the opportunity to divest herself of the current living husband by illegal means.... G. Includes the filigree as drawn H. The existing siding will remain intact. Option#1 Add an Entry to the living room as follows: -Frame an enclosed, insulated floor using 2x12, treated joists, with treated plywood underneath, insulated full, and covered with 3/4" Tongue in Groove plywood decking. -Frame the walls using 2x6, SPF studs, 16" on center, insulating full. Cover the exterior with 25/32" Biltrite and 3/4" Cedar lap siding to match the existing. Trim to match the current style. -Remove the existing siding and sheathing as needed -Prep the porch ceiling for enclosing by adding ventilation chutes, soffit venting and 12" fiberglass insulation and sheetrock backing boards -Furnish and install a 3'0" full view steel insulated entry door with handleset. Remove the existing storm door and reinstall on the new exterior door. -Sheetrock the ceiling and walls, taping smooth for paint. -Trim the doors and base to match the existing oak trim -Provide wiring for a switch and lite, a switch and exterior coach lite and relocate the buzzer. Customer to provide the lighting fixtures 10. Electrical. Includes the cost to provide openings and install surface lights on the ceiling, with a switch at the front door. Also includes two receptacles that could be switched on and off at the door(for Christmas lighting..) 11.
