The rights of individual on whom the data is stored or to be stored shall be as set forth in this section. Subd.2. Information required to be given individual. An individual asked to supply private or confidential data concerning himself shall be informed of: (a)the purpose and intended use of the requested data within the collecting state agency,political subdivision,or statewide system;(b) whether he may refuse or is legally required to supply the requested data;(c)any known consequence arising from his supplying or refusing to supply private or confidential data;and(d)the identity of other persons or entities authorized by state or federal law to receive the data. This requirement shall not apply when an individual is asked to supply investigative data,pursuant to section 13.82,subdivision 5,to a law enforcement officer. Access to data by individual. Construction 545 Indian Mound 1 • Wayzata, MN. 55391 Dear Lavern: The following is our proposal for a swimming pool to be installed at the 210 North Shore Dr. Orono, MN. For Mike and Pam Schultz. Imperial Pool will manufacture the pool; the pool will be an 18' by 36' Rectangle with the step across -the shallow end of the pool. The pool will be 3' deep in the shallow end and the deep end will be 61 . See drawing on the top_of this 'Page icor layout of bottom. The automatic cover will run in the concrete coping of the pool. :PLIMS AND SPECIFICATIONS: Dolphin Pool will furnish all plans and specifications covered by this quotation to meet Local building codes. TAS AND BUILDING PERMIT: Dolphin Pool will pay all local, state and federal taxes on the labor and materials for their work on this pool. Dolphin Pool will also furnish the building permit. POOL CONSTRUCTION: The entire pool will be constructed of a 14 gauge galvanized steel panel 42" in height. The panels are supported with a 12 gauge A-Frame brace and a deck brace located every 41 . A concrete bond beam, minimum of 6" is -then poured around the perimeter of the panels, base and braces . The structural- integrity of the galvanized steel panels is warranted for as long as the original customer owns the pool. The Liner is warranted for 15 years against manufacturing defects. The equipment such as the pump, filter and heater has warranties up to 5 years. POOL BASE: Minimum of 2" of a Vermiculite and Portland Cement base is troweled on the bottom of the Pool. VINYL, LINER: The liner will be Legacy Majestic Tile with a Blue Mirage Floor. The step will have the pattern on ® them. COPING: The coping will be Cantilevering Stamped Concrete. Dolphin Pool will provide aluminum forms for the coping. A concrete company doing the deck will pour the coping and the lids for the automatic cover. Dolphin has added the 10 brackets for supporting the lid at a cost of $55.00 each to this proposal. PLUMBING: Dolphin Pool will run all plumbing from the pool to the pool equipment. All pipe and fittings will be sized and approved by the Orono building department. DRAIN: Dolphin Pool will supply a backwash line to be run out from the equipment area into a suitable area in the back yard for backwashing and draining. WATER SERVICE: A standard garden hose will be used for filling and adding make-up water to the pool. ELECTRICAL SERVICE: Adequate electrical service and grounding facilities to the pool equipment area, wiring of filter pump, electronic ignition heater, under water lighting, grounding of pool, and all deck equipment will be by others. Dolphin Pools will secure pricing and will coordinate the electrical for the owner. The electrical should cost approximately $2,300.00 . SERVICE: Furnish and install the gas supply to pool heater, final connection, and permit and test line as per city code by others. Dolphin Pools will secure pricing and coordinate the gas service for the owner. The gas line will cost approximately $650.00. DECK AREA: No decking has been included as part of this proposal. Dolphin will do this work if requested at a charge of $12.00. EXCAVATION AND BACKFILLING: Dolphin Pool is not responsible for site preparation. Dolphin Pool will do the necessary excavation and back filling this pool. Any extra soil will be relocated to the front of the site or will be removed from the site. For Mike and Pam Schultz. The spa will be a model 509 Elite. SPA CONSTRUCTION: The Spa will be constructed a one piece Acrylic sheet reinforced with fiberglass. The color of the spa will be Starry Night Granite. The spa will be flush with the concrete decking around the pool area. SPA Tim iTY: The Emerald Spa has a 25-year Warranty of the Fiberglass Structure and a 5- year Warranty of the Acrylic Surface. The Installation and Equipment are warranted for 1 year. PLANS AND SPECIFICATIONS: Dolphin Pool will furnish all plans and specifications covered by this quotation to meet all State and Local building codes. TAXES AND BUILDING PERMIT: Dolphin Pool will pay all local, state and federal taxes on labor materials for their work on this Spa. The building permit if necessary will be included as part of the pool permit. HYDROTHERAPY JETS: The jet placement will be as seen in the perspective brochure. PLYING: The factory will do all the pre plumbing of the spa shell. Dolphin will run all the plumbing lines to and from the spa to the spa equipment. The Equipment is to be located by the house approximately 25' to 30, from the spa shell . ELECTRICAL SERVICE: Adequate electrical service and grounding to the spa and spa equipment area, wiring of spa control, filter pump, hydrotherapy pump, ozone, and grounding of deck equipment by others.
