Sec.13.04 RIGHTS OF SiJB,TECTS OF DATA
Subd. 1. Type of data. The rights of individual on whom the data is stored or to be stored shall be as set forth in this secaon.
Subd.2. Iaformation required to be given individual. An individual asked to suppiy private or confidendat data concerning himself
shall be informed of: (a)the purpose and intended use of the requested data within[he collecting state agency,poliacal subdivision,or statewide
rystem;(b)whether he may refuse or is legally required to suppiy the requested data;(c)any lmown consequence arising from his supplying or
refusing to supply private or confidendal dara;and(d)the idendry of ocher persons or enaaes authorized by sate or federal law to receive the data.
'Ihis requirem�nt shall not apply when an individual is asked to supply investi;adve data, pursuant to secdon 13.82, subdivision 5, to a law
enforcement officer.

Subd. 3. Access to data by individual. Upon request to a responsible authoriry,an individual shall be informed whether he is the

is the subject of stored private or public data on individuals shall be shown the data without any char�e to him and, if he desires,shall be informed
of the content and meanin�of that data. Afrer an individual has been shown the private data and informed of its meaning,the data need not be
disclosed to him for six months thereafrer unless a dispute or acaon pursuant to this secoon is pending or addirional data on the individual has been
collected or created. The responsible authoriry shall provide copies of the priva[e or public data upon request by the individual subject of the data.

disagreement. The responsible authoriry shall within 30 days either: (a)correct the dara found to be inaccurate or incomplete and attempt to nocify
past recipients of inaccurate or incomplete dara,including recipients named by the individual;or(b)nodfy the individual that he believes the data
to be correct. Data in dispute shall be disclosed oniy if the individual's statement of disagreement is included with the disclosed data.
The detzrminaaon of the respoasible authoriry may be appealzd pursuant to the provisions of the adminis�ative procedure act relating
to contested cases.

would like to use our product, called FE 125/�E �00. It is tnade by Foarr�
Enterprises Inc. of Plymouth, lVlN. I�cluded in this fax is tech data on this
product. The other information in this �ax is on the supplement to the IRC that
shows that this type of product has been approved for use in rimjoists, without
having to be fireproofed. There are 2 main criteria to consider. One is the
flammability. E1s the I�C supplement shows, a foam product used in this
application, must be a class � foam. A, class l foam has a Flame spread of 25
of less and a Smoke I�eveloped of 450 or 1ess. The Icynene product meets this
requirement, as does the Foarr� Enterprises product. �'he other requirement is
that the foa�n product must have a density of 1.5 to 2 lbs per cu. ft. �d'he Foam
En$erprises product meets this requirement. T'he Icynene does no�. Icynene is a
low-density material with a density of Iess than 1 lb. per cu. ft. Also, fihe
Icynene product is not a vapor barrier and the Foan� Enterprise product is a
vapor barrier.

TECHNICAL DATA SHEET
FE 125 / FE 800
FE 125 is a polyurethane rigid spray foam system designed for insulation of stud-wall cavities
and other applications where high yield and low density are desired. An approved thermal
barrier system must be applied over the sprayed insulation such as a '/Z" sheet rock or certain
types of spray applied cellulose. Building codes�cuRently require a 15 minute thermal barrier or
an ignition barrier be applied over the spray insulation depending upon the application. Check
with your local building official for final determination.
FE 125 may be applied in 1° to 4" passes. FE 125 is NOT �ecommended for use as an exterior
roof deck system due to the light density of this product. FE 125 should NOT be used in
refrigerated structures or where rapid change in temperature can occur. FE 125 is to be
applied to surfaces between 40°F to 120°F. Foam Enterprises' technical service personnel
should be consulted in all cases where application conditions are questionable. '
