Sec.13.04 RIGHTS OF SUBJECTS OF DATA
Subd. 1. Type of data. The rights of individual on whom[he data is stored or to be stored shall be as set forth in this secaon.
Subd.2. Information required to be given individual. An individual asked to supply private or confidential data concerning himself shall
be infom�ed of: (a)the purpose and intended use of the requested data within the collecting state agency,political subdivision,or statewide system;(b)
whether he may refuse or is legally required to supply the requested data;(c)any known consequence arising from his supplying or refusing to supply
private or confidential data;and(d)the identiry of other persons or enuues authorized by state or federal law w receive the data. This requirement shall
not apply when an individual is asked to supply inves6gative data,punuant to section 13.82,subdivision S, to a law enforcement officer.

Slibd.3. Access to data by mdividual. Upon request to a responsible authoriry,an individual shall be informed whether he is the subject of
stored data on individuals,and whether it is classified as public,private or confidendal. Upon his further request,an individual who is the subject of stored
private or public dara on individuals shall be shown the data without any charge to him and,if he desires,shall be informed of the content and meaning
of that data. After an individual has been shown the private data and informed of its meaning,the data need not be disclosed to him for six months
theceafter unless a dispute or action pursuant to this section is pending or additional data on the individual has been collected or created. The responsible
authoriry shall provide copies of the private or public data upon request by the individual subject of the data. The respoasible authoriry may require the

Subd.4. Procedure when data�not accurate or c�►plete. An individual may contest ihe accuracy or completeness of public or private data
conceming himself. To exercise this right,an individual shall notify in writing the responsible authoriry describing the nature of the disagreement. The
responsible authoriry shall within 30 days either. (a)correct the data found to be inaccurete or incomplete and attempt to notify past recipients of inaccurate
or incomplete dara,including recipients named by the individual;or(b)notify the individual that he believes the data to be correct. Data in dispute shall
be disclosed only if the individual's statement of disagreement is included with the discloszd data.
The de�rmination of the cesponsible authoriry may be appealed punuant w the provisions of the administrative procedure act relating to contested
cases.

DESIGN LOADS
A. Roof: 40 PSF live load (snow surcharge per Minnesota
code).
Roofing dead load assumed 3 PSF (Wood Shingles).
Assumed roof total design dead load 20 PSF.
B. Floor: Ground Floor: 50 PSF live load or 2000#/20in2�

SOIL PREPARATION AND FOUNDATIONS
A. Materials
1. Footings to bear on properly compacted fill or native
soil capable of sustaining an assumed bearing pressure of
2000 PSF in accordance with 1997 UBC Section 1805
(Class 3 material or better). Allowable bearing pressure to
be verified by an independent testing agency. If soil
appears to be of questionable quality or in a loose state,
contact Engineer before proceeding with the work.
2. All fill material to be granular soil or other
cohesionless material approved by the soil testing
agency.
3. Provide 4" granular subbase under slabs-on-grade ,
B. Installation Notes
1. Prior to installing foundations, soil to be field inspected
by an independent testing agency familiar with the site to
assure that the compaction specifications have been satisfied.
. Submit all inspection reports, material test reports, etc_ to
the Architect.
2. All topsoil is to be stripped from building and pavement
areas. All very loose sands and silt and clay soils also to
be stripped.
3. All fill material to be spread in maximum 6" loose lifts
and compacted with vibratory compactors to a minimum
98% of the maximum dry density as determined by ASTM
D698, Standard Proctor. Moisture content at the time of
compaction should be controlled to within +/- 3% of the
optimum moisture content.
, 4. Provide minimum 3'-6" soil cover for foundation frost

CONCRETE - SITE CAST
A. Design Code: ACI 318, ACI "Manual of Standard
Practice...", latest adoption.
B. Materials
1. Minimum concrete strength at 28 days (Pc)for
slabs-on-grade and walls to be 4000 psi. For footings,
3000 psi.
2. Concrete supplier to submit concrete mix designs to
Architect for all site cast concrete.
3. Furnish 5-7% air entraining agent for all concrete
exposed to freezing temperatures.
4. Reinforcing steel
a. Welded wire fabric- ASTM A185.
b. Deformed bars-ASTM A615, Grade 60.
5. Install slab on grade control joints in accordance with
PCA publication "Concrete Floors on Ground". '
C. Installation notes:
l. Support reinforcement only with CRSI recommended bar
supports(wood, brick, etc. not acceptable)or hang from
top of forms. Use supports with sand plates or horizontal
runners for slab,-on-grade reinforcement.
2. Concrete cover per ACI 318, latest adoption.
3. Sawcut slabs-on-�rade as soon as concrete may be worked
without unraveling(max. 6 hours after placement).
4. Place and securely tie footing dowels in position prior
to concrete placement(i.e.: do not embed dowels into
wet concrete).
CONCRETE-PRECAST PRESTRESSED
A. Design Code: ACI 318, latest adoption.
B. Concrete strength at 28 days(fc): 5000 psi minimum.
C. Supplier notes:
1. Precast, prestressed concrete shall be designed, •
fabricated, and erected according to the specifications
� of the ACI Code and the Prestressed Concrete Institute
Design Handbook, latest edition.
2. Precast plank shall be design�to withstand the
superimposed loads as indica.ted on plan.
3. Field welding of cast-in plates, etc.,to be performed _
only by certified welders in accordance vv�th AWS Dl.l,

4. Precast material to be manufactured in accordance with
the PCI Quality Control Manual, latest edition.

LUMBER
A. Design Code: "National Design Specification for Wood
Construction", latest edition.
B. Materials:
1. All members designated as "LVL" to have the following
minimum allowable design values: Fb=2.95 ksi,
Fv= 0.29 ksi, Fc(parallel to grain) = 3.18 ksi,
Fc(perpendicular to grain, parallel to glue line)= 1.02 ksi
and E=2000 ksi. Provide nailing/bolting connection for
multiple piece beams per manufacturer's specifications
2. All members designated as "PSL" column to have the following
minimum allowable design values: Fb=2.4 ksi,
Fc(parallel to grain)=2.5 ksi and E= 1800 ksi.
3. Wood Floor and Roof Trusses
a. Designed and manufactured in accordance with the
"Specification for Metal Plate Connected Wood
Trusses", latest edition.
b. Tross plates shall be of structural quality .

MISCELLANEOUS
A. Placement of inechanical units supported by roof or floor
structure is subject to the approval of the Engineer.
B. Verify all existing conditions and dimensions. Report all
conflicts in construction documents and actual conditions
to the Architect before proceeding with the affected work.
C. Contractor is responsible for bracing all structural elements,
without overstressing, until project is complete. Stockpiling
of materials on supported levels is at Contractor's own risk.
D. Contractor to temporarily shore earth retaining walls while
backfitling. Shores can be removed only after concrete
slab-on-grade is in place and has reached 70% of its design
strength and ground floor is installed and connected to wall.

SOIL PREPARATION AND FOUNDATIONS
A. Materials
1. Footings to bear on properly compacted fill or native
soil capable of susta�ning an assumed bearing pressure of
2000 PSF in accordance with 1997 UBC Section 1805
(Class 3 material or better). Allowable bearing pressure to
be verified by an independent testing a.�ency. If soil
appears to be of questionable quality or in a loose state,
contact Engineer before proceeding with the work.
2. All fill material to be granular soil or other
cohesionless material approved by the soil testing
agency.
3. Provide 4" granular subbase under slabs-on-grade
B. Installation Notes
1. Prior to installing foundations, soil to be field inspected
by an independent testing agency familiar with the site to
assure that the compaction specifications have been satisfied.
Submit all inspection reports, material test reports, etc. to
the Architect.
2. All topsoil is to be stripped from building and pavement
areas. All very loose sands and silt and clay soils also to
be stripped.
3. All fill material to be spread in maximum 6" loose lifts
and compacted with vibratory compactors to a minimum
98% of the maximum dry density as determined by ASTM
D698, Standard Proctor. Moisture content at the time of
compaction should be controlled to within+/_ 3% of the
optimum moisture content.
4. Provide minimum 3'-6" soil cover for foundation frost
protection. �

3. Sawcut slabs-on-grade as soon as concrete may be worked
without unraveling(max. 6 hours after placement).
4. Place and securely tie footing dowels in position prior
to concrete placement (i.e.: do not embed dowels into
